CURRENT POSITION OF BETA-ADRENERGIC BLOCKERS IN THE MANAGEMENT OF ARTERIAL HYPERTENSION

Sažetak. Razmotrene su trenutačne dvojbe oko uloge b-adrenergičkih blokatora u liječenju arterijske hipertenzije. Autor smatra da ti lijekovi doista nisu prikladni za starije hipertoničare, navlastito u primarnoj prevenciji moždanog udara. Dijabetogeni i aterogeni potencijal ovih lijekova nije osobit, a među postojećim predstavnicima nema bitnih razlika (atenolol možda ima stanovite male nedostatke). Ovi lijekovi za sada ostaju među antihipertenzivima prve linije za mlađe hipertoničare, a u slučaju apsolutnih indikacija ili nepodnošenja drugih antihipertenziva treba ih propisati svim hipertoničarima, kao što je Hrvatsko društvo za hipertenziju zaključilo još 1999. godine.

Summary. Discussed is the actual controversy concerning the role of b-adrenergic blocking agents in the treatment of arterial hypertension. The author argues that these drugs are not suitable for elderly hypertensive patients, particularly in primary stroke prevention. The diabetogenic and atherogenic potential of these agents is marginal, and there are no marked differences between the available  blockers (atenolol has possibly some slight disadvantages). However, for the time being these drugs remain in the first line antihypertensive armamentarium for the younger hypertensive patients; for compelling indications or intolerance of other antihypertensives should be prescribed to any patient, as Croatian Society of Hypertension concluded in 1999.
