<title>SA&#x017D;ETAK</title>
<p>Vi&#x0161;e od 200 papilomavirusa &#x010D;ovjeka (HPV) inficira stanice ljudskog epitela, od kojih su alfa i beta tipovi najopse&#x017E;nije prou&#x010D;avani i analizirani. Alfa-tipovi HPV-a primarno inficiraju epitel sluznice, a samo manji njihov dio povezan je s vi&#x0161;e od 600.000 karcinoma godi&#x0161;nje &#x0161;irom svijeta na razli&#x010D;itim anatomskim mjestima, posebno anogenitalnim i regijama glave i vrata. Najva&#x017E;niji karcinom uzrokovan alfa-tipovima HPV-a jest rak vrata maternice, koji je vode&#x0107;i uzrok smrti povezane s rakom kod &#x017E;ena u mnogim dijelovima svijeta. HPV kodiraju dva onkoproteina, E6 i E7, koji izravno sudjeluju u razvoju malignosti povezanih s HPV-om. Oni djeluju u sinergiji, ciljaju&#x0107;i razli&#x010D;ite stani&#x010D;ne puteve koji su uklju&#x010D;eni u regulaciju kontrole stani&#x010D;nog ciklusa, apoptoze i polariteta stanica. U ovom su pregledu istaknute biolo&#x0161;ke posljedice papilomavirusa koji djeluju na razli&#x010D;ite stani&#x010D;ne proteine na razli&#x010D;itim anatomskim mjestima u razvoju malignih tumora izazvanih HPV-om, s naglaskom na tumore glave i vrata.</p>

<title>SUMMARY</title>
<p>More than 200 human papillomaviruses (HPVs) infect human epithelial cells, and out of them alpha and beta types have been the most extensively investigated and analysed. Alpha HPVs primarily infect mucosal epithelia and only a small proportion of them is associated with more than 600,000 cancers per year worldwide at various anatomical sites, especially anogenital and head-and-neck region. Of these the central disease is cervical cancer, which is the leading cause of cancer-related deaths in women in numerous parts of the world. HPVs encode two oncoproteins, E6 and E7, which are directly involved in the development of HPV-mediated malignancies. They do this in cooperation by targeting various cellular pathways involved in the regulation of cell cycle control, of apoptosis and of cell polarity control networks. In this review, the biological consequences of HPV manipulating of various cellular proteins at diverse anatomical sites in the development of HPV-induced carcinogenesis are depicted, with focus on head and neck cancer.</p>

<p>Perzistentna infekcija papilomavirusima &#x010D;ovjeka (engl. HPV, <italic>human papillomavirus</italic>) glavni je &#x010D;imbenik rizika u razvoju brojnih zlo&#x0107;udnih bolesti na razli&#x010D;itim anatomskim mjestima (<xref ref-type="bibr" rid="r1"><italic>1</italic></xref>, <xref ref-type="bibr" rid="r2"><italic>2</italic></xref>): karcinom skvamoznih stanica vrata maternice, rodnice, vulve, penisa, anusa, rektuma i orofarinksa (<xref ref-type="table" rid="t1">Table 1</xref>). Me&#x0111;u njima se najvi&#x0161;e isti&#x010D;e rak vrata maternice koji poga&#x0111;a &#x017E;ene, uglavnom u zemljama u razvoju, i uzrokuje vi&#x0161;e od 570.000 karcinoma godi&#x0161;nje. <p>Iako je poznato da oko 200 vrsta HPV-a inficira ljude, samo je mali dio njih povezan s razvojem raka. (<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>) Papilomavirusi &#x010D;ine heterogenu skupinu virusa, koja je svrstana u porodicu <italic>Papillomaviridae</italic>. (<xref ref-type="bibr" rid="r2"><italic>2</italic></xref>) Tipovi HPV-a klasificirani su u pet rodova: alfa (&#x03B1;), beta (&#x03B2;), gama (&#x03B3;), mu (&#x03BC;) i nu (&#x03BD;), a rodovi &#x03B1; i &#x03B2; najintenzivnije su istra&#x017E;eni. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>) Skupina &#x03B1;-papilomavirusa sadr&#x017E;i tipove koji inficiraju epitele sluznice i koji su podijeljeni u tipove niskog rizika (engl. LR, <italic>low risk</italic>) i visokog rizika (engl. HR, <italic>high risk</italic>), na temelju njihove sposobnosti da uzrokuju rak. Infekcije LR tipovima HPV-a, od kojih su naj&#x010D;e&#x0161;&#x0107;i HPV-6 i HPV-11, rezultiraju benignim lezijama, dok su infekcije HR tipovima HPV-a povezane sa zlo&#x0107;udnim bolestima anogenitalnog podru&#x010D;ja te regije glave i vrata. Od njih, HPV-16 i HPV-18 uzrokuju pribli&#x017E;no 80% slu&#x010D;ajeva raka vrata maternice u svijetu, dok je preostalih 20% prete&#x017E;no povezano s drugim HR tipovima HPV-a, kao &#x0161;to su HPV-31, HPV-33, HPV-45, HPV-52 i HPV-58 (<xref ref-type="bibr" rid="r2"><italic>2</italic></xref>, <xref ref-type="bibr" rid="r3"><italic>3</italic></xref>) (<xref ref-type="table" rid="t2">Table 2</xref>). HPV-16 i HPV-18 su odgovorni za gotovo sve karcinome koji se mogu pripisati HPV-u u mu&#x0161;karaca. Zanimljivo je da je kod HPV-pozitivnih karcinoma glave i vrata, koji prvenstveno zahva&#x0107;aju orofarinks i javljaju se u tonzilama i na bazi jezika, HPV-16 najzastupljeniji, dok se ostali HR tipovi HPV-a rijetko otkrivaju. (<xref ref-type="bibr" rid="r5"><italic>5</italic></xref>, <xref ref-type="bibr" rid="r6"><italic>6</italic></xref>) Osim toga, HR HPV-pozitivni karcinomi tumora glave i vrata imaju bolju prognozu i pre&#x017E;ivljenje (<xref ref-type="bibr" rid="r7"><italic>7</italic></xref>, <xref ref-type="bibr" rid="r8"><italic>8</italic></xref>), zbog &#x010D;ega je utvr&#x0111;ivanje prisutnosti HPV-a iznimno va&#x017E;no za daljnje pra&#x0107;enje i lije&#x010D;enje oboljelih na ovom anatomskom sijelu. <title>Virusni &#x017E;ivotni ciklus</title>
<p>Papilomavirusi su mali DNA virusi bez ovojnice, ikosaedarske simetrije. (<xref ref-type="bibr" rid="r19"><italic>19</italic></xref>) Njihova je DNA dvolan&#x010D;ana, kru&#x017E;na i superuvijena, veli&#x010D;ine oko 7.900 parova baza (<xref ref-type="fig" rid="f1">Figure 1</xref>). Obavijena je kapsidom koja se sastoji od dvaju strukturnih proteina; glavni protein kapside L1 (55.000 daltona) &#x010D;ini 80% kapside, a protein L2 (70.000 daltona) ostatak. Promjer viriona je 52&#x2013;55 nm. Protein E1 neophodan je za episomalnu replikaciju virusne DNA, a protein E2 regulira transkripciju i replikaciju virusne DNA. Kod HPV-16 i HPV-18 protein E2 kontrolira aktivnost promotora za transkripciju gena E6 i E7. Produkti gena E6 i E7 glavni su transformiraju&#x0107;i proteini HR HPV-a. (<xref ref-type="bibr" rid="r20"><italic>20</italic></xref>) Protein E4 ima ulogu u destabilizaciji citokeratinske mre&#x017E;e te u kasnijem dijelu ciklusa poma&#x017E;e u osloba&#x0111;anju viriona iz stanice. Onkoprotein E5 se nalazi u stani&#x010D;noj membrani i poma&#x017E;e u prijenosu signala za &#x010D;imbenike rasta. (<xref ref-type="bibr" rid="r21"><italic>21</italic></xref>)</p>

<p>Virusna DNA HR HPV-a &#x010D;esto se ugra&#x0111;uje u genom doma&#x0107;ina (<xref ref-type="fig" rid="f1">Figure 1</xref>). Ugradnja u genom doma&#x0107;ina jest nasumi&#x010D;na, ali &#x010D;esto u blizini uobi&#x010D;ajenih lomljivih mjesta (<italic>common fragile sites</italic>) genoma, (<xref ref-type="bibr" rid="r22"><italic>22</italic></xref>) pa se u nekim slu&#x010D;ajevima virusna DNA ugra&#x0111;uje unutar stani&#x010D;nih gena ili u blizini stani&#x010D;nih onkogena (npr. c-<italic>myc</italic>). (<xref ref-type="bibr" rid="r23"><italic>23</italic></xref>) U dobro&#x0107;udnim o&#x0161;te&#x0107;enjima vezanima s HPV-infekcijama virusni genom postoji isklju&#x010D;ivo kao autonomno repliciraju&#x0107;i episom. Naj&#x010D;e&#x0161;&#x0107;e se ugra&#x0111;uju tipovi HPV-a 16, 18 i 45, iako ne uvijek. (<xref ref-type="bibr" rid="r24"><italic>24</italic></xref>) Tipovi HPV-a 31 i 33 ne ugra&#x0111;uju se gotovo uop&#x0107;e u genom doma&#x0107;ina, iako su vrlo u&#x010D;estali u raku vrata maternice. (<xref ref-type="bibr" rid="r25"><italic>25</italic></xref>) Prilikom ugradnje virusne DNA HR HPV-a naj&#x010D;e&#x0161;&#x0107;e dolazi do loma unutar gena E1 i E2, &#x010D;ime se remeti aktivnost tih gena, a posljedica ugradnje je povi&#x0161;ena i nekontrolirana ekspresija virusnih onkogena E6 i E7. (<xref ref-type="bibr" rid="r26"><italic>26</italic></xref>)</p>
<p>HR tipovi HPV-a inficiraju epitel sluznice i vjeruje se da virus ulazi kroz mikroo&#x0161;te&#x0107;enja i inficira bazalne stanice epitela (<xref ref-type="fig" rid="f2">Figure 2</xref>). &#x017D;ivotni ciklus virusa strogo ovisi o stani&#x010D;noj diferencijaciji keratinocita, glavnih ciljnih stanica virusa. Virus nema vlastiti replikacijski mehanizam i stoga ovisi o stani&#x010D;noj diobi i raslojavanju epitela doma&#x0107;ina, koji se odvija od bazalnih slojeva prema suprabazalnim slojevima, iskori&#x0161;tavaju&#x0107;i ovaj proces za replikaciju i stvaranje novih virusa. U ovom procesu onkoproteini E6 i E7 igraju presudnu ulogu. Naime, E6 uz pomo&#x0107; stani&#x010D;ne ubikvitin-ligaze E6AP ve&#x017E;e i razgra&#x0111;uje p53, (<xref ref-type="bibr" rid="r27"><italic>27</italic></xref>) dok onkoprotein E7 ve&#x017E;e i razgra&#x0111;uje pRB uz pomo&#x0107; ubikvitin-ligaze Cullin-2 kompleksa. (<xref ref-type="bibr" rid="r28"><italic>28</italic></xref>) Osim p53 i pRB, virusni onkoproteini dereguliraju i rad drugih bitnih stani&#x010D;nih proteina. Neki od najpoznatijih stani&#x010D;nih metaonkoproteina E6 uklju&#x010D;uju Bak, DLG1, SCRIBBLE, obitelj MAGI proteina, prokaspaza 8 i FADD, (<xref ref-type="bibr" rid="r20"><italic>20</italic></xref>, <xref ref-type="bibr" rid="r29"><italic>29</italic></xref>, <xref ref-type="bibr" rid="r30"><italic>30</italic></xref>) dok neke od najbitnijih stani&#x010D;nih metaonkoproteina E7 &#x010D;ine p107, p130 i PTPN14. (<xref ref-type="bibr" rid="r20"><italic>20</italic></xref>, <xref ref-type="bibr" rid="r30"><italic>30</italic></xref>) Kroz interakcije s ovim stani&#x010D;nim supstratima zajedni&#x010D;ko djelovanje ovih dvaju onkoproteina, usmjereno na razli&#x010D;ite stani&#x010D;ne puteve uklju&#x010D;ene u regulaciju kontrole stani&#x010D;nog ciklusa i apoptoze, omogu&#x0107;uje virusu da odr&#x017E;ava proliferaciju stanica u visokodiferenciranim suprabazalnim regijama, a time i replikaciju virusnog genoma. (<xref ref-type="bibr" rid="r19"><italic>19</italic></xref>, <xref ref-type="bibr" rid="r31"><italic>31</italic></xref>)</p>

<sec sec-type="other2">
<title>Uloga E6 i E7 u patogenezi</title>
<p>Normalni produktivni virusni &#x017E;ivotni ciklus HR tipova HPV-a visoko je reguliran i koordiniran proces. Me&#x0111;utim, u nekim slu&#x010D;ajevima, uglavnom tijekom trajne infekcije, virusna se DNA nasumi&#x010D;no ugra&#x0111;uje u genom doma&#x0107;ina (<xref ref-type="fig" rid="f1">Figure 1</xref>), &#x0161;to dovodi do stani&#x010D;ne imortalizacije i na kraju do maligne progresije. Ishod ovog procesa je kolaps virusne replikativne sposobnosti: ve&#x0107;ina virusnih gena se gubi, dok dva glavna virusna onkogena, E6 i E7, ostaju nekontrolirano izra&#x017E;eni (<xref ref-type="fig" rid="f2">Figure 2</xref>), pokre&#x0107;u&#x0107;i stani&#x010D;ne cikluse, daljnji napredak prema stani&#x010D;noj transformaciji i na kraju rezultiraju&#x0107;i razvojem raka. (<xref ref-type="bibr" rid="r32"><italic>32</italic></xref>, <xref ref-type="bibr" rid="r33"><italic>33</italic></xref>) Zanimljivo je da se proces virusne ugradnje doga&#x0111;a mnogo rje&#x0111;e kod orofaringealnih karcinoma nego kod raka vrata maternice. (<xref ref-type="bibr" rid="r34"><italic>34</italic></xref>)</p>
<p>Studije raka vrata maternice na transgenom mi&#x0161;jem modelu pokazale su da je E7 pove&#x0107;ao proliferaciju i broj kopija centrosoma i inducirao progresiju multifokalnih mikroinvazivnih karcinoma vrata maternice, dok je E6 pove&#x0107;ao broj kopija centrosoma i eliminirao uo&#x010D;ljivi protein p53, ali nije proizveo neoplaziju ili rak. Va&#x017E;an je podatak da je kombinacija obaju onkoproteina rezultirala pove&#x0107;anim brojem centrosoma i velikim, jako invazivnim karcinomima. (<xref ref-type="bibr" rid="r37"><italic>37</italic></xref>) Sli&#x010D;na su zapa&#x017E;anja zabilje&#x017E;ena i na mi&#x0161;jim modelima plo&#x010D;astih stanica raka glave i vrata (engl. HNSCC, <italic>head and neck squamous cell carcinoma</italic>). U njima se pokazalo da je E7 glavni transformiraju&#x0107;i onkoprotein, dok se &#x010D;ini da E6 vjerojatnije igra sekundarnu ulogu, gdje vi&#x0161;e doprinosi u kasnijim stadijima malignosti. (<xref ref-type="bibr" rid="r38"><italic>38</italic></xref>) Na temelju tih studija razvijen je model karcinogeneze izazvane HPV-om. Stoga se ova dva onkoproteina smatraju izvrsnim ciljevima za terapijsku intervenciju, a razumijevanje molekularnih mehanizama koji le&#x017E;e u osnovi njihovih funkcija presudno je za razvoj takvih antivirusnih terapija.</p>
</sec>
<sec sec-type="other3">
<title>Infekcije HR HPV-a u regiji glave i vrata</title>
<p>Ve&#x0107;ina tumora glave i vrata povezanih s infekcijom HR HPV javlja se u orofarinksu, poglavito tonzilama, dok je manji broj u usnoj &#x0161;upljini i larinksu. (<xref ref-type="bibr" rid="r39"><italic>39</italic></xref>&#x2013;<xref ref-type="bibr" rid="r42"><italic>42</italic></xref>) Kripte tonzila su oblo&#x017E;ene specijaliziranim plo&#x010D;astim i isprepletenim epitelom koji je infiltriran limfoidnim tkivom i bazalnim stanicama. (<xref ref-type="bibr" rid="r42"><italic>42</italic></xref>) Prisutnost bazaloidnih stanica sugerira osjetljivost na HPV-infekciju, a smatra se da je ovo i preferencijalna zona koja olak&#x0161;ava virusu izbjegavanje imunolo&#x0161;kog odgovora. Jednako je va&#x017E;no da ovo tkivo ne odr&#x017E;ava polarizaciju i povr&#x0161;insko sazrijevanje koje se nalazi u slojevitom plo&#x010D;astom epitelu. Dakle, u ovom bazaloidnom tkivu, zbog smanjenja polarizacije epitela te nedostatka barijere i bazalnih stanica, dolazi do pove&#x0107;anja mogu&#x0107;nosti za trajnu infekciju. Velika ve&#x0107;ina karcinoma povezanih s HPV-om na podru&#x010D;jima bazaloidnih stanica pozitivna je na HPV-16, s vrlo malim udjelom ostalih tipova HPV-a poput HPV tipova 18, 31 i 33, (<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>, <xref ref-type="bibr" rid="r43"><italic>43</italic></xref>&#x2013;<xref ref-type="bibr" rid="r46"><italic>46</italic></xref>) a razlozi za to jo&#x0161; nisu jasni. Zanimljivo je da je vrijeme izme&#x0111;u po&#x010D;etne infekcije i razvoja bolesti kod karcinoma glave i vrata znatno kra&#x0107;e nego kod anogenitalnih karcinoma. (<xref ref-type="bibr" rid="r47"><italic>47</italic></xref>) Sve to ukazuje na slo&#x017E;enu interakciju virusa i replikacijskog okru&#x017E;enja u podru&#x010D;ju tonzila.</p>
<p>Otprilike 70% karcinoma vrata maternice uzrokuju HPV-16 i HPV-18 (<xref ref-type="table" rid="t2">tablica 2</xref>), a mnoge stani&#x010D;ne mete onkoproteina E6 i E7 zajedni&#x010D;ke su za oba tipa virusa, dok razlike le&#x017E;e samo u stupnju interakcije. Suprotno tomu, gotovo ni&#x0161;ta nije poznato o njihovim stani&#x010D;nim supstratima u orofarinksu, koji mogu biti vrlo razli&#x010D;iti zbog slo&#x017E;enosti tkiva i okolnog okru&#x017E;enja. Potrebne su biokemijske i mehani&#x010D;ke studije virusnih onkogena i njihovih interakcijskih partnera u stanicama orofarinksa kako bi se razjasnile potencijalne razlike, kako u produktivnom &#x017E;ivotnom ciklusu virusa, tako i u malignosti izazvanoj ovim virusom. (<xref ref-type="bibr" rid="r30"><italic>30</italic></xref>)</p>

<title>Zaklju&#x010D;ak</title>
<p>Zanimljivo je da isti tip virusa, HPV-16, mo&#x017E;e pokazivati &#x200B;&#x200B;razli&#x010D;ita svojstva ovisno o mjestu zaraze. Svakako, &#x010D;ini se da je njegova aktivnost koja izaziva rak mnogo ve&#x0107;a u tkivima s transformacijskom zonom, poput vrata maternice, anusa ili tonzila, nego u onima gdje nije jasno predilekcijsko mjesto zaraze. (<xref ref-type="bibr" rid="r48"><italic>48</italic></xref>) Stoga, iako smo stekli ve&#x0107;e razumijevanje o na&#x010D;inu funkcioniranja virusnih onkoproteina, jo&#x0161; uvijek treba mnogo nau&#x010D;iti o njihovim aktivnostima u razli&#x010D;itim vrstama tkiva i njihovom okoli&#x0161;u.</p>
