<title>SA&#x017D;ETAK</title>
<sec><title>Uvod i cilj</title><p>U&#x010D;iteljice su u riziku od poreme&#x0107;aja glasa, koji negativno utje&#x010D;e na profesionalnu u&#x010D;inkovitost te kvalitetu &#x017E;ivota. Naju&#x010D;inkovitiji tretmanski ishodi povezani su s ranim otkrivanjem poreme&#x0107;aja glasa, za &#x0161;to se u&#x010D;inkovitim pokazuju upitnici samoprocjene, poput upitnika <italic>Screening Index for Voice Disorder</italic> (SIVD). Cilj ovog rada bio je kreirati i validirati njegovu hrvatsku ina&#x010D;icu (SIVD-HR) te ispitati njezin potencijal za predikciju bio-psiho-socijalnih posljedica poreme&#x0107;aja glasa kvantificiranih ve&#x0107; adaptiranim i validiranim upitnikom <italic>Indeks vokalnih te&#x0161;ko&#x0107;a</italic> (VHI-HR).</p></sec>
<sec><title>Ispitanici i metode</title><p>SIVD-HR sadr&#x017E;i dvanaest simptoma &#x010D;ija se izra&#x017E;enost samoprocjenjuje na Likertovoj ljestvici. Uvijek ili gotovo uvijek prisutan simptom boduje se jednim bodom, a ve&#x0107;i broj bodova ukazuje na ve&#x0107;i rizik poreme&#x0107;aja glasa. SIVD-HR je nakon informiranog pristanka ispunilo 109 u&#x010D;iteljica, podijeljenih u skupine s dijagnozom poreme&#x0107;aja glasa (VP) i bez dijagnoze poreme&#x0107;aja glasa (BVP) dobivene do unatrag dvije godine od vremena prikupljanja podataka.</p></sec>
<sec><title>Rezultati</title><p>Izra&#x017E;enost simptoma prosje&#x010D;no je procijenjena niskom, a njihov bio-psiho-socijalni utjecaj blagim, no u oba slu&#x010D;aja procjena VP skupine bila je zna&#x010D;ajno nepovoljnija. Barem jedan simptom (gotovo) uvijek osje&#x0107;a gotovo 50% u&#x010D;iteljica VP skupine i tek svaka osma u&#x010D;iteljica BVP skupine. Korelacije SIVD-HR s dobi, sta&#x017E;em i dnevnim radnim optere&#x0107;enjem nisu prona&#x0111;ene. Pokazalo se da je SIVD-HR pouzdan te da posjeduje diskriminacijsku, konvergentnu i prediktivnu valjanost. Podru&#x010D;je ispod krivulje operativnih karakteristika od 0,672 pri granici od dva ili vi&#x0161;e bodova te osjetljivost od 46,2% i specifi&#x010D;nost od 84% upu&#x0107;uju na klini&#x010D;ki nedostatnu to&#x010D;nost SIVD-HR u identifikaciji u&#x010D;iteljica s ranije postavljenom dijagnozom poreme&#x0107;aja glasa kao rizi&#x010D;nih i stoga na njegovu ograni&#x010D;enu iskoristivost. Me&#x0111;utim, SIVD-HR zna&#x010D;ajno i sna&#x017E;no predvi&#x0111;a izra&#x017E;enost bio-psiho-socijalnih posljedica vokalnih problema pa ga je uputno validirati na ve&#x0107;em broju vokalnih profesionalaca.</p></sec>
<sec><title>Zaklju&#x010D;ak</title><p>Rezultati sugeriraju neosvije&#x0161;tenost u&#x010D;iteljica o simptomima promjena kvalitete glasa i prihva&#x0107;anje njihovih posljedica kao otegotnih okolnosti posla, &#x0161;to upu&#x0107;uje na potrebu sustavnog obrazovanja u&#x010D;itelja o vokalnom zdravlju.</p></sec>

<title>SUMMARY</title>
<sec><title>Introduction and aim</title><p>Female teachers are at risk of voice disorders, which negatively affect professional effectiveness and quality of life. The most effective treatment outcomes are associated with early detection of voice disorders, for which self-assessment questionnaires, such as the Screening Index for Voice Disorder (SIVD), have been shown to be effective. The aim of this study was to create and validate its Croatian version (SIVD-HR) and to examine its potential for predicting the bio-psycho-social consequences of voice disorders quantified by the already adapted and validated Voice Handicap Index (VHI-HR).</p></sec>
<sec><title>Subjects and methods</title><p>The SIVD-HR contains 12 symptoms whose severity is self-assessed on a Likert scale. A symptom that is always or almost always present is scored with one point, and a higher number of points indicates a higher risk of voice disorders. The SIVD-HR was completed, after informed consent, by 109 female teachers, divided into groups with (VP) and without (BVP) diagnosis of voice disorder obtained up to two years from the time of data collection.</p></sec>
<sec><title>Results</title><p>The severity of symptoms was assessed as low on average, and their bio-psycho-social impact as mild, but in both cases the assessment of the VP group was significantly less favorable. At least one symptom is (almost)always felt by nearly 50% of teachers in the VP group and only one in eight teachers in the BVP group. Correlations with age, experience and daily workload were not found. The SIVD-HR was shown to be reliable and to have discriminant, convergent and predictive validity. The area under the <italic>receiver</italic> operating characteristic curve of 0.672 at a cut-off of two or more points, with sensitivity of 46.2% and specificity of 84%, indicate that the SIVD-HR is not accurate enough in identifying teachers with a previously diagnosed voice disorder as at risk, and therefore has limited clinical usefulness. However, the SIVD-HR significantly and strongly predicts the severity of the bio-psycho-social consequences of vocal problems, so it is advisable to validate it on a larger number of vocal professionals.</p></sec>
<sec><title>Conclusion</title><p>The results suggest that teachers are unaware of the symptoms of voice quality changes and accept their consequences as aggravating circumstances at work, which indicates the need for systematic education of teachers about vocal health.</p></sec>

<p>Poznato je da su u&#x010D;itelji zbog intenzivnoga vokalnog optere&#x0107;enja u pove&#x0107;anom riziku od razvijanja poreme&#x0107;aja glasa. (<xref ref-type="bibr" rid="r1"><italic>1</italic></xref>-<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>) Okosnica u&#x010D;iteljskog posla jest govorna interakcija s u&#x010D;enicima pa poreme&#x0107;aj glasa izravno negativno utje&#x010D;e na profesionalnu u&#x010D;inkovitost, na &#x0161;to se mogu nadovezati fizi&#x010D;ke, financijske te emocionalne posljedice koje kompromitiraju financijske, privatne i dru&#x0161;tvene aspekte &#x017E;ivota. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>, <xref ref-type="bibr" rid="r5"><italic>5</italic></xref>)</p>
<p>Pove&#x0107;an rizik od pojave poreme&#x0107;aja glasa kod u&#x010D;itelja rezultat je nepripremljenosti za dugotrajno govorenje (<xref ref-type="bibr" rid="r6"><italic>6</italic></xref>), okolinskih &#x010D;imbenika (pozadinske buke, lo&#x0161;e akustike i mikroklime u&#x010D;ionica) i osobnih reakcija na postavljene profesionalne zahtjeve (umora, anksioznosti i stresa koji nosi kontrola u&#x010D;enika i javno govorenje, emocionalne preoptere&#x0107;enosti uslijed radnog optere&#x0107;enja i poja&#x010D;ane potrebe za profesionalnim priznanjem). (<xref ref-type="bibr" rid="r7"><italic>7</italic></xref>, <xref ref-type="bibr" rid="r8"><italic>8</italic></xref>) S obzirom na to da u&#x010D;iteljski posao uglavnom obavljaju &#x017E;ene (<xref ref-type="bibr" rid="r9"><italic>9</italic></xref>), ote&#x017E;avaju&#x0107;i je &#x010D;imbenik i ve&#x0107;a sklonost &#x017E;ena razvoju poreme&#x0107;aja glasa, u odnosu na mu&#x0161;karce. (<xref ref-type="bibr" rid="r10"><italic>10</italic></xref>)</p>
<p>Sve navedeno povezano je s biomehani&#x010D;kim kompenzacijama (<xref ref-type="bibr" rid="r11"><italic>11</italic></xref>) koje proizvodnju glasa optere&#x0107;uju funkcionalno (prvenstveno kao vokalni zamor i mi&#x0161;i&#x0107;na tenzijska disfonija) te s posljedi&#x010D;nim strukturnim reakcijama tkiva na suprafiziolo&#x0161;ke fonacijske uvjete i fonotraume. (<xref ref-type="bibr" rid="r12"><italic>12</italic></xref>, <xref ref-type="bibr" rid="r13"><italic>13</italic></xref>)</p>
<p>Otkrivanje ranih znakova pojave poreme&#x0107;aja glasa i rana implementacija tretmanskih opcija od velikog su zna&#x010D;aja za uklanjanje ili ubla&#x017E;avanje vokalnih problema, odnosno prevenciju njihovog napredovanja prema klini&#x010D;ki ozbiljnijim posljedicama. (<xref ref-type="bibr" rid="r14"><italic>14</italic></xref>) Standardnim dijagnosti&#x010D;kim protokolom koji uklju&#x010D;uje medicinsku procjenu i evaluaciju kvalitete glasa te&#x0161;ko je otkriti najranije znakove njegovog poreme&#x0107;aja. Me&#x0111;utim, poreme&#x0107;aje glasa u visokorizi&#x010D;nim populacijama mogu&#x0107;e je neinvazivno, brzo, ekonomi&#x010D;no te rano otkrivati primjenom upitnika samoprocjene. (<xref ref-type="bibr" rid="r15"><italic>15</italic></xref>) Primjerice, instrument razvijen upravo u svrhu otkrivanja poreme&#x0107;aja glasa dok on jo&#x0161; nije uznapredovao jest upitnik <italic>Screening Index for Voice Disorder</italic> (SIVD). (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>) SIVD se sastoji od dvanaest simptoma poreme&#x0107;aja glasa &#x010D;ija samoprocjena izra&#x017E;enosti ukazuje na rizik od postojanja poreme&#x0107;aja glasa. Upitnik ima odgovaraju&#x0107;e psihometrijske karakteristike (<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>, <xref ref-type="bibr" rid="r17"><italic>17</italic></xref>, <xref ref-type="bibr" rid="r18"><italic>18</italic></xref>) te mo&#x017E;e biti iskori&#x0161;ten za procjenu utjecaja razli&#x010D;itih terapijskih tehnika na kvalitetu glasa (<xref ref-type="bibr" rid="r19"><italic>19</italic></xref>, <xref ref-type="bibr" rid="r20"><italic>20</italic></xref>) i u planiranju budu&#x0107;ih terapijskih postupaka i prevenciji pogor&#x0161;anja simptoma. (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>)</p>
<p>Zbog me&#x0111;unarodne usporedbe klini&#x010D;kih studija &#x010D;esto je potrebno izraditi nekoliko ina&#x010D;ica specifi&#x010D;nih mjerenja na razli&#x010D;itim jezicima (<xref ref-type="bibr" rid="r21"><italic>21</italic></xref>), kako bi se rezultati mogli uspore&#x0111;ivati, a zaklju&#x010D;ci generalizirati (<xref ref-type="bibr" rid="r22"><italic>22</italic></xref>). Jezi&#x010D;ne/kulturolo&#x0161;ke adaptacije originalnih instrumenata za samoprocjenu kvalitete glasa dobrodo&#x0161;le su i zbog &#x010D;injenice da prisutnost i percipirana izra&#x017E;enost simptoma poreme&#x0107;aja glasa mo&#x017E;e biti razli&#x010D;ita u razli&#x010D;itim jezicima. (<xref ref-type="bibr" rid="r23"><italic>23</italic></xref>)</p>
<p>S obzirom na to da SIVD jo&#x0161; nije adaptiran na hrvatski jezik, prvi cilj rada bio je ispitati dijagnosti&#x010D;ke metrijske karakteristike hrvatske ina&#x010D;ice (SIVD-HR). Drugi cilj odnosio se na procjenu potencijala SIVD-HR za predikciju rezultata upitnika <italic>Indeks vokalnih te&#x0161;ko&#x0107;a</italic> (VHI-HR) (<xref ref-type="bibr" rid="r24"><italic>24</italic></xref>), budu&#x0107;i da istra&#x017E;ivanja sugeriraju kako u&#x010D;iteljice &#x010D;esto pokazuju nisku razinu zabrinutosti za kvalitetu glasa i simptome do&#x017E;ivljavaju kao uobi&#x010D;ajenu posljedicu rada. (<xref ref-type="bibr" rid="r24"><italic>24</italic></xref>&#x2013;<xref ref-type="bibr" rid="r26"><italic>26</italic></xref>) Poseban naglasak stavljen je na razliku izme&#x0111;u SIVD-HR, koji se temelji isklju&#x010D;ivo na simptomima, i VHI-HR, koji obuhva&#x0107;a opisne &#x010D;estice i procjenjuje psihosocijalni utjecaj poreme&#x0107;aja glasa. <italic>(</italic><xref ref-type="bibr" rid="r27"><italic>27</italic></xref><italic>&#x2013;</italic><xref ref-type="bibr" rid="r32"><italic>32</italic></xref><italic>)</italic> Usporedba s VHI-HR omogu&#x0107;ila je ispitivanje sklonosti sudionica da podcjenjuju zna&#x010D;aj vokalnih simptoma u vlastitoj samoprocjeni.</p>
<sec sec-type="other1">
<title>Metode</title>
<sec>
<title>Sudionici</title>
<p>Slijede&#x0107;i populaciju na kojoj je validiran izvorni SIVD te uzimaju&#x0107;i u obzir da je u hrvatskim &#x0161;kolama prisutna izrazita feminizacija u&#x010D;iteljske struke (<xref ref-type="bibr" rid="r9"><italic>9</italic></xref>), sudionice ovog istra&#x017E;ivanja bile su &#x017E;ene u dobi od 23 do 63 godine (M = 40,72; SD = 11,16) zaposlene u osnovnim &#x0161;kolama, s radnim sta&#x017E;om od 1 do 42 godine (M = 16,06; SD = 12,07) i tjednim nastavnim optere&#x0107;enjem od 5 do 40 sati (M = 22,17; SD = 2,63).</p>
<p>Sudionice su regrutirane na predavanjima o poreme&#x0107;ajima glasa i vokalnoj higijeni u organizaciji &#x017E;upanijskih stru&#x010D;nih vije&#x0107;a na podru&#x010D;ju grada Zagreba, &#x010D;ija su vodstva bila upoznata da je prikupljanje demografskih podataka i podataka o zdravlju glasa sastavni dio ovih stru&#x010D;nih aktivnosti. Sudionicama je prije po&#x010D;etka predavanja obja&#x0161;njen cilj istra&#x017E;ivanja i na&#x010D;in njegovog provo&#x0111;enja, nakon &#x010D;ega je svaka dobrovoljno mogla odlu&#x010D;iti o nastavku suradnje potpisom informiranog pristanka. Na sudjelovanje su se odlu&#x010D;ile sve u&#x010D;iteljice (N=109) koje su poha&#x0111;ale spomenute radionice u nastavnoj godini 2023./2024. te su im podijeljeni materijali koje su trebale ispuniti: upitnik s demografskim podatcima i podatcima o zdravlju glasa te SIVD-HR i VHI-HR. Prikupljanje podataka provo&#x0111;eno je sukladno na&#x010D;elima Deklaracije iz Helsinkija iz 2000. godine i njenim dopunama iz 2002. i 2004. godine. (<xref ref-type="bibr" rid="r33"><italic>33</italic></xref>)</p>
<p>Dijagnosti&#x010D;ka svojstva testa uobi&#x010D;ajeno je provjeravati usporedbom sa zlatnim standardom, odnosno standardiziranim dijagnosti&#x010D;kim postupkom. (<xref ref-type="bibr" rid="r34"><italic>34</italic></xref>) Zato su sudionice istra&#x017E;ivanja bile pitane jesu li u zadnje dvije godine dobile dijagnozu poreme&#x0107;aja glasa (<xref ref-type="bibr" rid="r35"><italic>35</italic></xref>), na &#x0161;to su pozitivno odgovorile &#x010D;ak 52 sudionice (47,7%), dok preostalih 57 sudionica (52,3%) nije prijavilo dijagnozu poreme&#x0107;aja glasa u protekle dvije godine. Sudionice koje su odgovorile pozitivno formirale su skupinu s vokalnim problemima (grupa VP), a one koje nisu imale dijagnozu formirale su skupinu bez vokalnih problema (grupa BVP). Grupa VP je potom zamoljena da naknadno slu&#x017E;benu dijagnozu, postavljenu od strane specijalista otorinolaringologa, upi&#x0161;e u <italic>online</italic> upitnik putem poveznice poslane na njihove adrese e-po&#x0161;te. Na taj na&#x010D;in dijagnoze nisu mogle biti povezane s pojedinom osobom, ve&#x0107; su samo mogle biti deskriptivno analizirane: dijagnozu funkcionalnog poreme&#x0107;aja glasa navelo je dvadeset u&#x010D;iteljica, vokalne nodule osamnaest u&#x010D;iteljica, LPR pet u&#x010D;iteljica, mi&#x0161;i&#x0107;nu tenzijsku disfoniju &#x010D;etiri u&#x010D;iteljice, polip na glasnicama tri u&#x010D;iteljice te Reinkeov edem dvije u&#x010D;iteljice.</p>

<sec>
<title>Mjerni instrumenti</title>
<p>Demografskim upitnikom prikupljeni su podatci o dobi, duljini radnog sta&#x017E;a i satima tjednoga radnog optere&#x0107;enja te informacije o dijagnozi poreme&#x0107;aja glasa, odnosno iskustvu s problemima s glasom.</p>
<p>Za ranu identifikaciju rizika poreme&#x0107;aja glasa kori&#x0161;ten je SIVD (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>) koji se sastoji od dvanaest simptoma: promuklost, gubitak glasa, pucanje glasa, glas niske frekvencije, sekret, suhi ka&#x0161;alj, ka&#x0161;alj sa sekrecijom, bol pri govorenju, bol pri gutanju, sekrecija u grlu, suho grlo i napet govor. SIVD daje mogu&#x0107;nost ocjene u&#x010D;estalosti pojave svakog simptoma na Likertovoj ljestvici s odgovorima &#x201E;nikada&#x201C; i &#x201E;ponekad&#x201C; kodiranima kao 0 (simptom nije prisutan) i odgovorima &#x201E;gotovo uvijek&#x201C; i &#x201E;uvijek&#x201C; kodiranima kao 1 (simptom je prisutan). U bodovanju se raspon kre&#x0107;e od 0 do 12, a rezultati od 5 i vi&#x0161;e potencijalno su rizi&#x010D;ni za nastanak poreme&#x0107;aja glasa te se preporu&#x010D;uje konzultacija s logopedom ili otorinolaringologom u svrhu procjene glasovnih te&#x0161;ko&#x0107;a. (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>)</p>
<p>Adaptacija izvornog SIVD-a (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>) zapo&#x010D;ela je nezavisnim prijevodom dvanaest simptoma od strane tri logopeda s preko deset godina klini&#x010D;kog iskustva s poreme&#x0107;ajima glasa. Prijevod je zatim pregledao nezavisan iskusni stru&#x010D;njak otorinolaringolog iz jednoga zagreba&#x010D;koga klini&#x010D;koga bolni&#x010D;kog centra. U&#x010D;injena je usporedba svih mi&#x0161;ljenja te je, s obzirom na slaganje uklju&#x010D;enih stru&#x010D;njaka oko prvog prijevoda simptoma sadr&#x017E;anih u SIVD-u, bilo mogu&#x0107;e kreirati prvu hrvatsku ina&#x010D;icu upitnika. Drugi nezavisan iskusni stru&#x010D;njak otorinolaringolog iz istoga zagreba&#x010D;koga klini&#x010D;koga bolni&#x010D;kog centra preveo je hrvatsku ina&#x010D;icu SIVD-a natrag na engleski jezik te je u&#x010D;injena usporedba tog prijevoda i originalnog SIVD-a. Dva su se prijevoda slagala u potpunosti pa daljnje modifikacije hrvatske ina&#x010D;ice SIVD-a (SIVD-HR) nisu bile potrebne i ta je ina&#x010D;ica kori&#x0161;tenja za prikupljanje podataka (<xref ref-type="fig" rid="f2">Figure 2</xref>).</p>

<p>VHI-HR (<xref ref-type="bibr" rid="r24"><italic>24</italic></xref>) hrvatska je adaptacija jednog od naj&#x0161;ire kori&#x0161;tenih upitnika u podru&#x010D;ju poreme&#x0107;aja glasa &#x2013; upitnika <italic>Indeks vokalnih te&#x0161;ko&#x0107;a</italic> (<xref ref-type="bibr" rid="r36"><italic>36</italic></xref>) &#x2013; razvijenog za kvantifikaciju pacijentove percepcije problema s glasom i njihovog utjecaja na svakodnevni &#x017E;ivot. Sadr&#x017E;i trideset pitanja raspore&#x0111;enih u tri podskale: funkcionalnu, fizi&#x010D;ku i emocionalnu. Odgovori se boduju na sljede&#x0107;i na&#x010D;in: &#x201E;nikad&#x201C; kao 0, &#x201E;gotovo nikad&#x201C; kao 1, &#x201E;ponekad&#x201C; kao 2, &#x201E;gotovo uvijek&#x201C; kao 3, a &#x201E;uvijek&#x201C; kao 4. Ukupni rezultat kre&#x0107;e se od 0 do 120 bodova.</p>

<sec>
<title>Metode obrade podataka</title>
<p>Odgovori na SIVD-HR i VHI-HR bodovani su i zbrojeni u sumarne varijable.</p>
<p>Nakon deskriptivne analize u&#x010D;injena je Spearmenova korelacijska analiza sumarnih rezultata SIVD-HR i VHI-HR u svrhu ispitivanja konvergentne valjanosti SIVD-HR. Neparametrijskim Mann-Withneyevim U-testom testirane su razlike u rezultatima na SIVD-HR izme&#x0111;u grupe VP i grupe BVP, kako bi se ispitala njegova diskriminacijska valjanost.</p>
<p>Za ispitivanje valjanosti upitnika SIVD-HR kori&#x0161;ten je generalizirani linearni model koji je testirao predikciju pripadnosti grupi VP ili BVP putem dobivenih ukupnih rezultata. Iskoristivost SIVD-HR u predvi&#x0111;anju psihosocijalnih posljedica poreme&#x0107;aja glasa kvantificiranih s VHI-HR ispitana je linearnom regresijskom analizom.</p>
<p>Pouzdanost je provjerena ra&#x010D;unanjem Cronbachove alfe, a ponovljivost rezultata ra&#x010D;unanjem Intraklasnog korelacijskog koeficijenta (ICC) kao omjer varijance izme&#x0111;u sudionika i ukupne varijance. (<xref ref-type="bibr" rid="r37"><italic>37</italic></xref>) Vrijednosti Cronbachove alfe kategorizirane su kao prihvatljive (&gt;0,7), dobre (&gt;0,8) ili odli&#x010D;ne (&gt;0,9). (<xref ref-type="bibr" rid="r38"><italic>38</italic></xref>) Vrijednosti ICC kategorizirane su kao umjerene (0,5 <bold>&#x2013;</bold> 0,75), dobre (0,75 &#x2013; 0,9) ili odli&#x010D;ne (&gt;0,9). (<xref ref-type="bibr" rid="r39"><italic>39</italic></xref>)</p>
<p>Analizom krivulje operativnih karakteristika (ROC) definirano je podru&#x010D;je ispod krivulje (AUC) te valjanost, pouzdanost, osjetljivost i specifi&#x010D;nost SIVD-HR u identifikaciji pojedinaca u riziku od problema s glasom. Mogu&#x0107;nost diskriminacije sudionika pomo&#x0107;u vrijednosti AUC kategorizirana je kao: nepostoje&#x0107;a (&#x2264;0,5), vrlo lo&#x0161;a (0,5 &#x2013; 0,6), lo&#x0161;a (0,6 &#x2013; 0,7), prihvatljiva (0,7 &#x2013; 0,8), odli&#x010D;na (0,8 &#x2013; 0,9) i izvanredna (&#x2265;0,9). (<xref ref-type="bibr" rid="r40"><italic>40</italic></xref>)</p>
<p>Statisti&#x010D;ka analiza napravljena je u programu IBM SPSS Statistics (<italic>IBM Corporation</italic>, Armonk, NY, SAD), a odabrana razina zna&#x010D;ajnosti za sve statisti&#x010D;ke postupke bila je p &lt; 0,05.</p>
</sec>
</sec>
<sec sec-type="other2">
<title>Rezultati</title>
<p>Deskriptivni rezultati pokazuju da su sudionice u prosjeku prijavile 0,84 simptoma (SD 1,65), koji su se kretali u rasponu od 0 do 8 simptoma. U grupi VP sudionice su u prosjeku prijavile 1,50 simptom (SD 2,10) u rasponu od 0 do 8 simptoma, a u grupi BVT tek 0,25 simptoma (SD 0,69), u rasponu od 0 do 4 simptoma. Ove kvantitativne podatke dopunjuje <xref ref-type="fig" rid="f1">Figure 1</xref>, koja sadr&#x017E;i frekvencije ukupnih rezultata SIVD-HR u obje grupe i iz kojih je vidljiva razlika izme&#x0111;u grupa u ukupnom broju simptoma &#x010D;ije se pojavljivanje percipira &#x201E;gotovo uvijek&#x201C; i &#x201E;uvijek&#x201C;.</p>

<p>Na <xref ref-type="fig" rid="f2">Figure 2</xref> prikazane su frekvencije odabira pojedinih simptoma SIVD-HR u obje grupe sudionica (VP i BVP). Kao naju&#x010D;estaliji simptomi u grupi VP prijavljeni su promuklost, sekret, sekrecija u grlu, suho grlo i napet govor, a grupa BVP tako&#x0111;er je &#x010D;esto prijavljivala probleme sa sekretom te suhi ka&#x0161;alj, &#x010D;ak podjednako kao i grupa VP, a me&#x0111;u njihovim &#x010D;e&#x0161;&#x0107;im simptomima jo&#x0161; se mogu vidjeti i suho grlo i promuklost.</p>
<p>Ukupni prosje&#x010D;ni rezultati na VHI-HR, kao i na tri njegove podskale &#x2013; funkcionalnoj (VHI-HRf), fizi&#x010D;koj (VHI-HRp) i emocionalnoj (VHI-HRe), u &#x010D;itavom uzorku ukazuju na blage bio-psiho-socijalne posljedice vokalnih problema. Me&#x0111;utim, iz <xref ref-type="table" rid="t1">Table 1</xref> vidljivo je da su prosje&#x010D;ne vrijednosti grupe VP na SIVD-HR i na tri od &#x010D;etiri mjere samprocjene utjecaja vokalnih te&#x0161;ko&#x0107;a na kvalitetu &#x017E;ivota (VHI-HR, VHI-HRp i VHI-HRe) bile statisti&#x010D;ki zna&#x010D;ajno vi&#x0161;e od onih u grupi BVP. Navedeno ukazuje na smisleno grupiranje sudionica prema kriteriju postavljene dijagnoze poreme&#x0107;aja glasa unatrag dvije godine.</p>

<p><xref ref-type="table" rid="t2">Table 2</xref> prikazuje zna&#x010D;ajne rezultate Spearmenove korelacijske analize, kojom je ispitivan odnos izme&#x0111;u prosje&#x010D;nih rezultata na SIVD-HR te na VHI-HR za sve sudionice, odnosno za VP i BVP grupe. Utvr&#x0111;ene su statisti&#x010D;ki zna&#x010D;ajne, umjerene i pozitivne korelacije izme&#x0111;u SIVD-HR i drugih mjera samoprocjene (VHI-HR i njegove tri podskale), &#x0161;to dokazuje konvergentnu valjanost SIVD-HR. Korelacije SIVD-HR s dobi, sta&#x017E;em i dnevnim radnim optere&#x0107;enjem nisu bile izra&#x017E;ene u stupnju niti zna&#x010D;ajne ni u jednoj grupi sudionica.</p>

<p>Kako se mo&#x017E;e vidjeti iz <xref ref-type="table" rid="t3">Table 3</xref>, rezultati primjene generaliziranoga linearnog modela potvr&#x0111;uju prediktivnu valjanost SIVD-HR: Waldova statistika je zna&#x010D;ajna na odabranoj razini zna&#x010D;ajnosti, a s obzirom na to da je kod za grupu BVP bio 0, negativni predznak procjene ukazuje da sudionice s vi&#x0161;im ukupnim rezultatom SIVD-HR imaju manju vjerojatnost pripadnosti grupi BVP, odnosno imaju ve&#x0107;u vjerojatnost prisutnosti vokalnih te&#x0161;ko&#x0107;a. Analiza primjerenosti modela, prikazana u istoj tablici, potvr&#x0111;uje njegovu primjerenost za predikciju pripadnosti grupi VP ili BVP, budu&#x0107;i da su skalirane vrijednosti devijance i Personovog HI2 niske.</p>

<p>Cronbachova alfa za SIVD-HR iznosila je 0,80, a ICC 0,80 uz 95-postotni interval pouzdanosti (95% CI) izme&#x0111;u 0,74 i 0,85, indiciraju&#x0107;i dobru internu konzistenciju skale. Korigirane korelacije pojedinih &#x010D;estica s ukupnim rezultatom kretale su se izme&#x0111;u 0,39 (simptom &#x201E;glas niske frekvencije&#x201C;) i 0,62 (simptom &#x201E;suhi ka&#x0161;alj&#x201C;), a Cronbachova alfa uz izostavljanje pojedine &#x010D;estice kretala se izme&#x0111;u 0,77 (simptom &#x201E;suhi ka&#x0161;alj&#x201C;) i 0,82 (simptom &#x201E;glas niske frekvencije&#x201C;).</p>
<p>Analiza krivulje operativnih karakteristika (ROC) (<xref ref-type="fig" rid="f3">Figure 3</xref>) pokazala je da je najbolja proporcija pozitivnih i la&#x017E;no pozitivnih rezultata izme&#x0111;u jednog i dva prijavljena simptoma, odnosno izme&#x0111;u ukupno jednog i ukupno dva ostvarena boda. Sukladno tomu, probirne osobine SIVD-HR analizirane su za bodovnu granicu od &#x2265;2 kao indikaciju rizika poreme&#x0107;aja glasa, pri kojoj je podru&#x010D;je ispod krivulje ili ukupna to&#x010D;nost predikcije skale (AUC) bilo 0,672 (95% CI 0,568-0,775), osjetljivost 0,462, a specifi&#x010D;nost 0,158. Dakle, rezultat od dva ili vi&#x0161;e bodova (dva ili vi&#x0161;e simptoma) na SIVD-HR smatran je indikacijom pove&#x0107;anog rizika od razvoja problema glasa, odnosno indikacijom za detaljniji pregled zdravlja glasa, pri &#x010D;emu je op&#x0107;a to&#x010D;nost skale u diskriminaciji pojedinaca sa samoprocijenjenim vokalnim problemima i bez njih, u usporedbi sa slu&#x010D;ajnom odlukom &#x2013; slaba. (<xref ref-type="bibr" rid="r40"><italic>40</italic></xref>)</p>

<p>To&#x010D;nost klasifikacije sudionika u grupu VP ili BVP pomo&#x0107;u rezultata SIVD-HR mo&#x017E;e se analizirati iz <xref ref-type="table" rid="t4">Table 4</xref>. Sudionice bez postavljene dijagnoze poreme&#x0107;aja glasa u protekle dvije godine klasificirane su u grupu BVP s visokih 84% to&#x010D;nosti, dok je to&#x010D;nost klasifikacije sudionica s postavljenom dijagnozom poreme&#x0107;aja glasa u protekle dvije godine na slabih 46%, &#x0161;to je ukupnu to&#x010D;nost klasifikacije svelo na 66%.</p>

<p>U <xref ref-type="table" rid="t5">Table 5</xref> nalaze se rezultati regresijske analize primijenjene za ispitivanje iskoristivosti SIVD-HR u predvi&#x0111;anju psihosocijalnih posljedica glasovnih te&#x0161;ko&#x0107;a kvanitificiranih s VHI-HR. Koeficijent multiple korelacije R iznosi 0,614, koeficijent determinacije (R2) iznosi 0,337, a korigirani koeficijent determinacije 0,371. Analiza varijance potvr&#x0111;uje da je regresijski model zna&#x010D;ajan (F (1,107) = 64,802; p = 0,000). Snaga doprinosa rezultata SIVD-HR predikciji rezultata VHI-HR, utvr&#x0111;ena pomo&#x0107;u statisti&#x010D;ki zna&#x010D;ajne vrijednosti (p = 0,000) nestandardiziranoga regresijskog koeficijenta (B) od 5,976 upu&#x0107;uje da se sa svakim prijavljenim simptomom na SIVD-HR rezultat na VHI-HR u prosjeku pove&#x0107;a za oko pet bodova. Stoga, sigurno je re&#x0107;i da ukupni rezultati SIVD-HR zna&#x010D;ajno i sna&#x017E;no (beta = 0,614, p = 0,000) sudjeluju u predvi&#x0111;anju izra&#x017E;enosti psihosocijalnih posljedica vokalnih problema, i to na na&#x010D;in da se 33,7% varijabilnosti rezultata na VHI-HR mo&#x017E;e objasniti rezultatima SIVD-HR.</p>

<p>Kako bi se utvrdila mogu&#x0107;a pristranost regresijskog modela, istra&#x017E;eno je postoji li u podatcima problem multikolinearnosti, autokorelacije te ekstremnih sirovih vrijednosti reziduala. Multikolinearnosti u modelu nije bilo, budu&#x0107;i da je vrijednost tolerancije u analizi parcijalnih korelacija iznosila 1, odnosno bila ve&#x0107;a od 0,10. (<xref ref-type="bibr" rid="r41"><italic>41</italic></xref>) Autokorelacije nije bilo, s obzirom na to da je izra&#x010D;unata vrijednost Durbin-Watsonovog testa bila blizu 2 (<xref ref-type="bibr" rid="r42"><italic>42</italic></xref>), odnosno iznosila je 2,153. Sirovi reziduali nisu odstupali vi&#x0161;e od &#x00B1;3 standardne devijacije, a analizom Cookovih udaljenosti me&#x0111;u rezidualima utvr&#x0111;ena je maksimalna vrijednost od 0,133, &#x0161;to je manje od gornje dozvoljene granice od 0,5. (<xref ref-type="bibr" rid="r41"><italic>41</italic></xref>) Prema tome, rezultati regresijske analize nisu pod utjecajem pojedinih ekstremnih rezultata.</p>
</sec>
<sec sec-type="other3">
<title>Rasprava</title>
<p>Temeljem rezultata deskriptivne analize podataka mogu&#x0107;e je uvidjeti da u&#x010D;iteljice na SIVD-HR u prosjeku prijavljuju manje od jednog simptoma poreme&#x0107;aja glasa, a one me&#x0111;u njima koje ve&#x0107; imaju povijest problema s glasom u prosjeku prijavljuju tek ne&#x0161;to vi&#x0161;e: jedan do dva simptoma poreme&#x0107;aja glasa. Njihova je percepcija utjecaja tih simptoma na kvalitetu &#x017E;ivota (rezultati VHI-HR) u prosjeku tek &#x2013; blaga, no ipak je zna&#x010D;ajno sna&#x017E;nija me&#x0111;u sudionicama s iskustvom poreme&#x0107;aja glasa, nego kod onih bez tog iskustva.</p>
<p>Analiza frekvencija ukupnih rezultata SIVD-HR u grupi VP i BVP, to jest broja simptoma poreme&#x0107;aja glasa &#x010D;ija se prisutnost percipira &#x201E;gotovo uvijek&#x201C; ili &#x201E;uvijek&#x201C; u pojedinoj grupi, otkriva da je modalna vrijednost, koja je u obje skupine iznosila 0, u grupi VP odabrana u 53,8% slu&#x010D;ajeva, a u grupi BVP &#x010D;ak u 84,2% slu&#x010D;ajeva. Ovaj podatak upu&#x0107;uje na to da je gotovo svaka druga osoba kojoj je u protekle dvije godine dijagnosticiran poreme&#x0107;aj glasa prijavila jedan ili vi&#x0161;e simptoma povezanih s poreme&#x0107;ajem glasa koji se pojavljuju &#x201E;gotovo uvijek&#x201C; ili &#x201E;uvijek&#x201C;, dok je isto u&#x010D;inila otprilike tek svaka osma osoba bez dijagnoze poreme&#x0107;aja glasa.</p>
<p>Frekvencije odabira pojedinih simptoma za skupinu u&#x010D;iteljica s glasovnim problemima djelomi&#x010D;no se poklapaju s rezultatima ranijih istra&#x017E;ivanja (<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>), koja izvje&#x0161;tavaju da su me&#x0111;u u&#x010D;iteljicama promuklost i suho&#x0107;a grla naj&#x010D;e&#x0161;&#x0107;e pritu&#x017E;be vezane za vokalno zdravlje. &#x010C;est odabir simptoma &#x201E;sekret&#x201C; znakovit je jer se nakupljanje sekreta mo&#x017E;e dovesti u vezu s upalom glasnica uslijed hiperfunkcionalne produkcije glasa (<xref ref-type="bibr" rid="r43"><italic>43</italic></xref>), karakteristi&#x010D;nom za u&#x010D;iteljice (<xref ref-type="bibr" rid="r12"><italic>12</italic></xref>), zatim s pojavom GERB-a i/ili LPR-a (<xref ref-type="bibr" rid="r44"><italic>44</italic></xref>) koji mogu pratiti stres i anksioznost na poslu (<xref ref-type="bibr" rid="r7"><italic>7</italic></xref>) te s infekcijama gornjih di&#x0161;nih puteva, koje su me&#x0111;u u&#x010D;iteljicama &#x010D;e&#x0161;&#x0107;e nego me&#x0111;u ostalom populacijom (<xref ref-type="bibr" rid="r45"><italic>45</italic></xref>).</p>
<p>Neo&#x010D;ekivan rezultat je izostanak korelacije SIVD-HR s dobi, sta&#x017E;em i dnevnim radnim optere&#x0107;enjem sudionica, koji se smatraju jakim i stabilnim doprinositeljima pove&#x0107;anju rizika razvoja simptoma poreme&#x0107;aja glasa me&#x0111;u u&#x010D;iteljicama. (<xref ref-type="bibr" rid="r14"><italic>14</italic></xref>)</p>
<p>Rezultati statisti&#x010D;ke analize podataka pokazuju da je SIVD-HR pouzdan (Cronbachova alfa od 0,80) te da posjeduje diskriminacijsku, konvergentnu i prediktivnu valjanost. Vrijednost Cronbachove alfe za SIVD-HR manja je od ranije prijavljenih vrijednosti od 0,86 (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>), odnosno 0,87 (<xref ref-type="bibr" rid="r35"><italic>35</italic></xref>). Ipak, uz ICC vrijednost od 0,80, interna konzistetnost SIVD-HR od 0,80 mo&#x017E;e se smatrati dobrom. Uz povoljne rezultate provjere valjanosti skala se mo&#x017E;e smatrati prihvatljivom za daljnju ograni&#x010D;enu uporabu te potencijalno korisnim klini&#x010D;kim insrumentom, nakon rigoroznije validacije na ve&#x0107;em uzorku sudionika. Me&#x0111;utim, analiza ROC krivulje ukazala je da su probirne karakteristike SIVD-HR nepovoljne u odnosu na podatke za originalan upitnik prema kojima je AUC 0,826, osjetljivost 0,94 i specifi&#x010D;nost 0,664 pri granici od pet ili vi&#x0161;e bodova kao indikacijom rizika postojanja poreme&#x0107;aja glasa (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>), to jest na tursku adaptaciju za koju je AUC 0,758, osjetljivost 0,552, a specifi&#x010D;nost 0,860 pri granici od &#x010D;etiri ili vi&#x0161;e bodova kao indikacijom rizika postojanja poreme&#x0107;aja glasa (<xref ref-type="bibr" rid="r35"><italic>35</italic></xref>). AUC od 0,672, izra&#x010D;unat za SIVD-HR pri granici od dva ili vi&#x0161;e bodova kao indikacijom rizika postojanja poreme&#x0107;aja glasa ne opravdava njegovu &#x0161;iroku klini&#x010D;ki primjenu jer sugerira (pre)skromnu to&#x010D;nost u diferenciranju osoba s poreme&#x0107;ajem glasa i bez poreme&#x0107;aja glasa. (<xref ref-type="bibr" rid="r46"><italic>46</italic></xref>) Osjetljivost SIVD-HR od 0,462 upu&#x0107;uje na to da se samoprocjenom simptoma poreme&#x0107;aja glasa me&#x0111;u hrvatskim u&#x010D;iteljicama dobiva preko 50% la&#x017E;no negativnih rezultata. Puno povoljniji podatak jest specifi&#x010D;nost SIVD-HR, prema kojoj se vi&#x0161;e od 84% sudionica bez dijagnoze zaista i identificiraju kao osobe koje nisu u riziku od poreme&#x0107;aja glasa. Drugim rije&#x010D;ima, rezultat od dva ili vi&#x0161;e bodova na SIVD-HR ostvarilo je samo 34,62% sudionica u grupi VP, ali je &#x010D;ak 84,20% sudionica u grupi BVP ostvarilo manje od dva boda. Ovi podatci upu&#x0107;uju na &#x010D;injenicu da nedostatak ve&#x0107;e to&#x010D;nosti u identifikaciji u&#x010D;iteljica s ranije postavljenom dijagnozom poreme&#x0107;aja glasa kao rizi&#x010D;nih (onih koje posti&#x017E;u dva ili vi&#x0161;e bodova na SIVD-HR) mo&#x017E;e biti posljedica njihove niske percepcije u&#x010D;estalosti simptoma. Budu&#x0107;i da je glas kulturalni konstrukt, percepcija poreme&#x0107;aja glasa i njegovih posljedica pod utjecajem je kulturalnih uvjerenja, odnosno kulturom uvjetovanog identiteta i sustava vrijednosti. (<xref ref-type="bibr" rid="r47"><italic>47</italic></xref>) Primjerice, u anketi studenata zavr&#x0161;nih godina sveu&#x010D;ili&#x0161;nog studija primarnoga obrazovanja u Hrvatskoj ustanovljeno je da su obilje&#x017E;ja idealnih u&#x010D;itelja, pored jasno&#x0107;e i razgovijetnosti govora, i u te&#x010D;nosti izra&#x017E;avanja, bogatstvu rje&#x010D;nika te uporabi pravilnoga knji&#x017E;evnog jezika, odnosno da su prili&#x010D;no &#x0161;ira od same kvalitete glasa. (<xref ref-type="bibr" rid="r48"><italic>48</italic></xref>) Izgleda da budu&#x0107;i u&#x010D;itelji idealnog govornika najprije povezuju s govorni&#x0161;tvom i ukupnim govorni&#x010D;kim umije&#x0107;em, to jest tek posredno s kvalitetom glasa. Istovremeno se u 65% slu&#x010D;ajeva smatraju tek djelomi&#x010D;no dobrim govornicima (<xref ref-type="bibr" rid="r49"><italic>49</italic></xref>), &#x0161;to isti&#x010D;e njihovu usmjerenost na sadr&#x017E;ajne i prezentacijske aspekte govora, prije nego na njegova akusti&#x010D;ka obilje&#x017E;ja.</p>
<p>Mogu&#x0107;e je i da je manja osjetljivost SIVD-HR, u odnosu na navode drugih autora, (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>, <xref ref-type="bibr" rid="r35"><italic>35</italic></xref>) odraz prosje&#x010D;no ve&#x0107;eg prihva&#x0107;anja utjecaja profesionalnih zahtjeva na kvalitetu glasa me&#x0111;u sudionicama ovog istra&#x017E;ivanja. Prema nekim autorima, preko 70% ispitanih u&#x010D;itelja u Hrvatskoj smatra kako je bavljenje u&#x010D;iteljskom profesijom uzrokovalo pote&#x0161;ko&#x0107;e u glasu koje osje&#x0107;aju, ali ih tek 17% tra&#x017E;i stru&#x010D;nu pomo&#x0107; zbog tog problema. (<xref ref-type="bibr" rid="r50"><italic>50</italic></xref>) Ovaj podatak mo&#x017E;e usmjeriti na zaklju&#x010D;ak da u&#x010D;iteljice u Hrvatskoj simptome poreme&#x0107;aja glasa smatraju uobi&#x010D;ajenom posljedicom svojeg posla te im zato pridaju manju va&#x017E;nost, a tomu u prilog ide i nalaz ovog istra&#x017E;ivanja o nezna&#x010D;ajnim korelacijama izme&#x0111;u SIVD-HR te dobi, sta&#x017E;a i dnevnoga radnog optere&#x0107;enja, usprkos epidemiolo&#x0161;kim potvrdama da me&#x0111;u u&#x010D;iteljicama rizik razvoja poreme&#x0107;aja glasa raste s duljim trajanjem karijere. (<xref ref-type="bibr" rid="r51"><italic>51</italic></xref>) Me&#x0111;utim, samo u 5% slu&#x010D;ajeva, me&#x0111;u svim sudionicama istra&#x017E;ivanja, odgovor &#x201E;nikada&#x201C; bio je odabran za svih dvanaest ili za jedanaest simptoma sadr&#x017E;anih u SIVD-HR, &#x0161;to ukazuje da gotovo sve sudionice imaju povremeno iskustvo s barem dva simptoma poreme&#x0107;aja glasa koje nije bodovano, a to je sukladno i blagim (<xref ref-type="bibr" rid="r52"><italic>52</italic></xref>) bio-psiho-socijalnim posljedicama glasovnih te&#x0161;ko&#x0107;a na koje ukazuje njihov prosje&#x010D;ni VHI. Ukupni rezultati SIVD-HR zna&#x010D;ajno i sna&#x017E;no (beta = 0,614, p = 0,000) sudjeluju u predvi&#x0111;anju izra&#x017E;enosti bio-psiho-socijalnih posljedica vokalnih problema, i to na takav na&#x010D;in da se 33,7% varijabilnosti rezultata na VHI-HR mo&#x017E;e objasniti rezultatima SIVD-HR. Dakle, sudionice pokazuju tendenciju da simptome vokalnih problema i njihove bio-psiho-socijalne posljedice samoprocijene dosljedno (&#x010D;e&#x0161;&#x0107;u pojavu simptoma prati ve&#x0107;a izra&#x017E;enost njihovih posljedica). Blaga osjetljivost na simptome i posljedice poreme&#x0107;aja glasa mo&#x017E;e biti potaknuta istim inherentnim razlogom: stvaranjem navike na pojavu simptoma poreme&#x0107;aja glasa, odnosno slabljenjem senzibiliteta za promjene kvalitete glasa i/ili razvojem kompenzacijskih mehanizama, uslijed &#x010D;ega sudionice tendiraju umanjiti njihov zna&#x010D;aj.</p>
<p>Pri dono&#x0161;enju zaklju&#x010D;aka o rezultatima ovog istra&#x017E;ivanja u obzir treba uzeti i njegova ograni&#x010D;enja. Utvr&#x0111;ena skromna probirna obilje&#x017E;ja SIVD-HR svakako trebaju biti provjerena na reprezentativnijem uzorku sudionika, a tu daljnju provjeru opravdavaju pouzdanost i valjanost potvr&#x0111;ene u ovom radu. Mogu&#x0107;e je pretpostaviti da bi se na ve&#x0107;em broju sudionika dobile jasnije informacije o odnosu samopercepcije simptoma poreme&#x0107;aja glasa i dobi, radnog sta&#x017E;a, dnevnoga radnog optere&#x0107;enja te vrste radnog ambijenta (vrti&#x0107;, osnovna i srednja &#x0161;kola). Uputno je u budu&#x0107;im istra&#x017E;ivanjima ispitati odnos rezultata SIVD-HR i: spola; razli&#x010D;itih dijagnoza poreme&#x0107;aja glasa i razli&#x010D;itih tretmana poreme&#x0107;aja glasa (poput konzumacije lijekova); razli&#x010D;itih profila vokalnih profesionalaca (na primjer glumci, sve&#x0107;enici, sportski treneri, radijski i televizijski voditelji i drugi); razli&#x010D;itih &#x017E;ivotnih navika (na primjer konzumacije alkohola, odre&#x0111;enih vrsta hrane ili nikotina, psihofizi&#x010D;kog stanja); skupine sudionika s razli&#x010D;itim stupnjevima informiranosti o higijeni glasa; drugih mjernih instrumenata (<italic>Indeksa vokalnih te&#x0161;ko&#x0107;a</italic>, <italic>Indeksa vokalnog zamora</italic>) i metoda procjene glasa (objektivnih i perceptivnih), a kako bi se stekao kriti&#x010D;an broj raznovrsnih informacija bitnih za organizaciju i provo&#x0111;enje klini&#x010D;kog rada i preventivnih aktivnosti me&#x0111;u vokalnim profesionalcima.</p>
</sec>
<sec sec-type="other4">
<title>Zaklju&#x010D;ak</title>
<p>Epidemiolo&#x0161;ki gledaju&#x0107;i, (<xref ref-type="bibr" rid="r51"><italic>51</italic></xref>) me&#x0111;u u&#x010D;iteljicama koje su sudjelovale u ovom istra&#x017E;ivanju poreme&#x0107;aji glasa zastupljeni su u o&#x010D;ekivanom omjeru. Usprkos tomu, one prosje&#x010D;no prijavljuju malu u&#x010D;estalost pojave simptoma poreme&#x0107;aja glasa i njihove blage posljedice. Utjecaj poreme&#x0107;aja glasa na profesionalnu u&#x010D;inkovitost te na fizi&#x010D;ke, dru&#x0161;tvene, financijske, emocionalne i mentalne aspekte &#x017E;ivota u&#x010D;itelja desetlje&#x0107;ima je znanstveno dokumentiran te je postao op&#x0107;e znanje, &#x0161;to upu&#x0107;uje na malu vjerojatnost da su u&#x010D;iteljice u Hrvatskoj jednostavno otpornije ili bolje pripremeljene na profesionalna optere&#x0107;enja rizi&#x010D;na za zdravlje njihovog glasa, nego njihove kolegice u drugim zemljama. Umjesto toga, razumnije je zaklju&#x010D;iti da dobiveni podatci odra&#x017E;avaju neosvije&#x0161;tenost sudionica o promjenama u kvaliteti glasa i prihva&#x0107;anje njihovih posljedica kao otegotne okolnosti njihovog posla. Navedeno upu&#x0107;uje na potrebu sustavnog obrazovanja u&#x010D;itelja o vokalnom zdravlju, ranim simptomima njegove ugroze te tehnikama vokalne higijene i drugim tehnikama samopomo&#x0107;i kojima se poreme&#x0107;aji glasa u ovoj visokorizi&#x010D;noj populaciji mogu prevenirati ili ubla&#x017E;iti te time blagotvorno djelovati na radnu u&#x010D;inkovitost i kvalitetu &#x017E;ivota.</p>
