<title>SA&#x017D;ETAK</title>
<sec><title>Ciljevi</title><p>Bolesnici s reumatoidnim artritisom (RA) imaju pove&#x0107;an rizik mortaliteta i morbiditeta zbog kardiovaskularnih bolesti (KVB). Bolesnici s RA imaju 50 &#x2013; 60% ve&#x0107;u vjerojatnost da &#x0107;e razviti KVB nego op&#x0107;a populacija. U ovom smo istra&#x017E;ivanju ispitali kako pu&#x0161;enje kao tradicionalni &#x010D;imbenik rizika za kardiovaskularne bolesti utje&#x010D;e na rizik od kardiovaskularnih bolesti pacijenata s RA.</p></sec>
<sec><title>Metode</title><p>Provedeno je osmogodi&#x0161;nje prospektivno klini&#x010D;ko kohortno istra&#x017E;ivanje u Klini&#x010D;kome bolni&#x010D;kom centru (KBC) Osijek putem planiranih ambulantnih vizita na koje su pozivani ispitanici. Istra&#x017E;ivanje je uklju&#x010D;ivalo 201 ispitanika, 124 s reumatoidnim artritisom (RA) kao ispitivanu skupinu i 77 s osteoartritisom (OA) kao kontrolnu skupinu. Istra&#x017E;ivanje je zavr&#x0161;ilo 137 ispitanika, 82 s RA i 55 s OA. Prosje&#x010D;na dob na po&#x010D;etku istra&#x017E;ivanja ispitanika u skupini s RA bila je 59,78 (37 &#x2013; 81) godina, dok je ona u skupini s OA bila 64,23 (27 &#x2013; 80) godine. Tijekom istra&#x017E;ivanja preminulo je 58 sudionika (41 s RA i 17 s OA). Usporedili smo prevalenciju pu&#x0161;enja i njezinu povezanost s rizikom od kardiovaskularnih bolesti pacijenata s RA.</p></sec>
<sec><title>Rezultati</title><p>U skupini s RA pu&#x0161;enje je zna&#x010D;ajno bilo vi&#x0161;e nego u skupini s OA (p = 0,0001). Tijekom razdoblja pra&#x0107;enja trendovi prevalencije pu&#x0161;enja u obje skupine nisu se zna&#x010D;ajnije promijenili. Pu&#x0161;enje je bilo povezano s pove&#x0107;anim rizikom od kardiovaskularnih bolesti u skupini s RA i za sada&#x0161;nje i za biv&#x0161;e pu&#x0161;a&#x010D;e (p = 0,028 i p = 0,016).</p></sec>
<sec><title>Zaklju&#x010D;ak</title><p>Na&#x0161;i rezultati upu&#x0107;uju na to da pu&#x0161;a&#x010D;i imaju ve&#x0107;u vjerojatnost da &#x0107;e razviti RA i KVB, a razlog na kojem se temelji trend nastavka pu&#x0161;enja cigareta u bolesnika s RA ostaje nejasan. Potrebna su daljnja istra&#x017E;ivanja kako bi se razjasnio trend nastavka pu&#x0161;enja u bolesnika s RA.</p></sec>

<title>SUMMARY</title>
<sec><title>Objectives</title><p>Patients with rheumatoid arthritis (RA) are at an increased risk of mortality and morbidity due to cardiovascular disease (CVD). RA patients are 50&#x2013;60% more likely than the overall population to develop CVD. In this investigation, we examined how smoking affected RA patients&#x2019; CVD risk.</p></sec>
<sec><title>Methods</title><p>An eight-year follow-up prospective, clinical cohort research was conducted, at the University Hospital Osijek through special outpatient visits to which participants were invited. The study included 201 participants, 124 with rheumatoid arthritis (RA) as investigation group and 77 with osteoarthritis (OA) as control group. The investigation was finished for 137 participants, 82 with RA and 55 with OA. The average participants age at the beginning of the investigation for RA group was 59.78 years, while that of the OA group was 64.23 years. During the course of the investigation, 58 participants (41 with RA and 17 with OA) passed away. We compared the prevalence of smoking and how it relates to RA patients&#x2019; risk for cardiovascular disease.</p></sec>
<sec><title>Results</title><p>In the RA group, smoking was more common than in the OA group (p=0.0001). During the follow-up period, trends in the prevalence of smoking in both groups did not significantly alter. Smoking was associated with an increased risk of CVD in the RA group for both current and former smokers (p=0.028 and p=0.016, respectively).</p></sec>
<sec><title>Conclusion</title><p>Our findings imply that smokers are more likely to develop RA and CVD, the reason underlying the trend of continued cigarette smoking in patients with RA remains unclear. Further studies are needed to clarify the trend of continued smoking in patients with RA.</p></sec>

<p>Kardiovaskularne bolesti (KVB) produljuju trajanje hospitalizacije, broj posjeta lije&#x010D;nicima te rizike obolijevanja i umiranja kod bolesnika s kroni&#x010D;nim upalnim bolestima, posebice reumatoidnim artritisom (RA). Osim toga, KVB pove&#x0107;ava tro&#x0161;kove lije&#x010D;enja pacijenata s RA i predstavlja zna&#x010D;ajan financijski izdatak za zdravstveni sustav. (<xref ref-type="bibr" rid="r1"><italic>1</italic></xref>, <xref ref-type="bibr" rid="r2"><italic>2</italic></xref>) Ve&#x0107;ina studija i metaanaliza pokazuje da bolesnici s RA imaju 50 &#x2013; 60% ve&#x0107;i rizik od razvoja KVB-a u usporedbi s op&#x0107;om populacijom. (<xref ref-type="bibr" rid="r1"><italic>1</italic></xref>-<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>) Osim toga, bolesnici s RA imaju pove&#x0107;ani rizik od smrtnosti zbog KVB-a, osobito zbog kroni&#x010D;ne ishemijske bolesti srca, akutnog infarkta miokarda i akutne cerebrovaskularne bolesti. (<xref ref-type="bibr" rid="r2"><italic>2</italic></xref>, <xref ref-type="bibr" rid="r3"><italic>3</italic></xref>)</p>
<p>Za procjenu rizika od kardiovaskularne smrti i nefatalnih doga&#x0111;aja u op&#x0107;oj populaciji na temelju &#x010D;imbenika rizika koristi se nekoliko alata. Europsko kardiolo&#x0161;ko dru&#x0161;tvo (ESC) (2021.) preporu&#x010D;uje upotrebu algoritma za predvi&#x0111;anje rizika sustavne procjene koronarnog rizika verzija 2 (SCORE 2) za procjenu rizika od kardiovaskularnih bolesti u europskoj populaciji. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>) ESC preporu&#x010D;uje da se za pacijente s RA procjena relativnog rizika dobivena iz rezultata SCORE 2 pomno&#x017E;i s 1,4 za mu&#x0161;karce i 1,5 za &#x017E;ene ako su ispunjena dva od tri sljede&#x0107;a kriterija: trajanje RA &gt;10 godina, pozitivan reumatoidni faktor (RF) ili anticitrulinska protutijela (anti-CCP) ili prisutnost izvanzglobnih manifestacija RA. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>) Tradicionalni &#x010D;imbenici rizika za KVB uklju&#x010D;uju promjenjive &#x010D;imbenike kao &#x0161;to su arterijska hipertenzija, dislipidemija, inzulinska rezistencija ili dijabetes, pu&#x0161;enje cigareta i niska tjelesna aktivnost, te nepromjenjive &#x010D;imbenike kao &#x0161;to su genetsko naslije&#x0111;e, rasa, dob i spol. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>) Iako se postotak pu&#x0161;a&#x010D;a u zapadnim razvijenim zemljama smanjuje, jo&#x0161; uvijek je prili&#x010D;no visok u isto&#x010D;noj Europi i jo&#x0161; uvijek ima zna&#x010D;ajan utjecaj na kardiovaskularni rizik i smrtnost od KVB-a. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>, <xref ref-type="bibr" rid="r5"><italic>5</italic></xref>)</p>
<p>U ovoj prospektivnoj kohortnoj klini&#x010D;koj studiji istra&#x017E;ivali smo prevalenciju pu&#x0161;enja cigareta u bolesnika s RA i analizirali odnos izme&#x0111;u pu&#x0161;enja cigareta i KVB-a u bolesnika s dijagnosticiranim RA (ispitivana skupina) i osteoartritisom (OA) (kontrolna skupina), kod bolesnika koji prethodno nisu imali dijagnosticiranu KVB ili simptomatsko zatajenje srca (HF), uz usporedbu s op&#x0107;om populacijom Hrvatske.</p>
<sec sec-type="other1">
<title>Materijali i metode</title>
<p>Za ovo istra&#x017E;ivanje odabrani su ispitanici iz Zavoda za reumatologiju KBC-a Osijek. Od uklju&#x010D;enih bolesnika, 124 s dijagnosticiranim RA bilo je uklju&#x010D;eno u ispitnu skupinu, dok je 77 s OA raspore&#x0111;eno u kontrolnu skupinu. Pacijenti su odabrani iz baze podataka Zavoda (nije bila digitalna u vrijeme probira). Ru&#x010D;no smo pretra&#x017E;ili bazu podataka i uklju&#x010D;ili sve prihvatljive pacijente prema kriterijima uklju&#x010D;ivanja i isklju&#x010D;ivanja navedenim u nastavku. Vrijeme za uklju&#x010D;ivanje trajalo je godinu dana. Ispitanici su pra&#x0107;eni u prosjeku osam godina i 4&#x00B1;3 mjeseca izme&#x0111;u 2008./9. i 2016./17.</p>
<p>Ispitanici i njihove obitelji upu&#x0107;eni su da prijave sve promjene u svom zdravstvenom statusu, uklju&#x010D;uju&#x0107;i pojavu kardiovaskularnih bolesti ili novih &#x010D;imbenika rizika za kardiovaskularne bolesti, bilo koju drugu bolest, kirur&#x0161;ki zahvat ili pogor&#x0161;anje RA (dostavljen je popis) telefonom, e-po&#x0161;tom ili osobnim posjetom u bilo kojem trenutku tijekom istra&#x017E;ivanja. Kriteriji uklju&#x010D;ivanja u ovo istra&#x017E;ivanje bili su sljede&#x0107;i: dijagnoza RA i OA za bolesnike u ispitivanoj, odnosno kontrolnoj skupini, od strane reumatologa prema klasifikacijskim kriterijima Ameri&#x010D;koga reumatolo&#x0161;kog dru&#x0161;tva (American College of Rheumatology), (<xref ref-type="bibr" rid="r6"><italic>6</italic></xref>) davanje potpisanoga informiranog pristanka te stalno prebivali&#x0161;te u jednoj od pet &#x017E;upanija isto&#x010D;ne Hrvatske. Kriteriji za isklju&#x010D;enje bili su sljede&#x0107;i: dijagnoza bilo kojeg oblika KVB-a ili simptomatskoga sr&#x010D;anog zatajivanja prije uklju&#x010D;ivanja, odbijanje sudjelovanja nakon inicijalne (uklju&#x010D;ne) vizite, trajni prestanak boravka u jednoj od pet &#x017E;upanija isto&#x010D;ne Hrvatske tijekom razdoblja istra&#x017E;ivanja ili nedostupnost tijekom ispitivanja. Dijagnosticirane KVB za isklju&#x010D;enje bile su: bilo koji tip infarkta miokarda ili angine pektoris, zna&#x010D;ajna koronarna bolest dokazana dijagnosti&#x010D;kim testovima, simptomatska ili asimptomatska ishemijska kardiomiopatija, mo&#x017E;dani udar ili prolazni ishemijski napad, akutna ili kroni&#x010D;na disekcija ili aneurizma aorte, penetrantni ulkus aorte ili intramuralni hematom, simptomatska ili asimptomatska periferna arterijska bolest. Svi pacijenti kod kojih se sumnjalo na KVB na temelju zabilje&#x017E;enih ili otkrivenih znakova, simptoma ili pretraga tako&#x0111;er su bili isklju&#x010D;eni.</p>
<p>Svi uklju&#x010D;eni ispitanici (ispitna i kontrolna skupina) pro&#x0161;li su po&#x010D;etnu (uklju&#x010D;nu) i zavr&#x0161;nu vizitu (ako su bili dostupni), dok su ispitanici u ispitivanoj skupini bili dolazili na godi&#x0161;nje vizite za procjenu aktivnosti RA. Prilikom prve vizite, svi ispitanici su ispunili sveobuhvatni pripremljeni obrazac (CRF): upitnik, fizikalni pregled i laboratorijske pretrage. Vizite su obuhva&#x0107;ale ispunjavanje upitnika koji je napisan na hrvatskom jeziku i sadr&#x017E;i pitanja o cjelokupnom zdravstvenom stanju; fizikalni pregled, uklju&#x010D;uju&#x0107;i mjerenje krvnog tlaka tri puta u razmacima od deset minuta (za analizu je kori&#x0161;tena prosje&#x010D;na vrijednost), te&#x017E;ine, visine te opsega struka i bokova; procjenu op&#x0107;eg zdravlja i intenziteta boli pomo&#x0107;u vizualno analognih ljestvica; popunjavanje <italic>Disease Activity Score 28</italic> s izmjerenim vrijednostima C-reaktivnog proteina (DAS28-CRP) (samo za RA skupinu), indeks te&#x017E;ine OA za &#x0161;ake (samo za OA skupinu), Lequesneov indeks za OA kuka i koljena (samo za OA skupinu) i hrvatsku verziju Upitnika za procjenu zdravlja (HAQ) obje skupine; elektrokardiografiju u&#x010D;injenu pomo&#x0107;u 12-kanalnog elektrokardiografa te uzimanje uzorka venske krvi za laboratorijsku analizu brzine sedimentacije eritrocita i C-reaktivnog proteina, ukupnog kolesterola, lipoproteina visoke gusto&#x0107;e, lipoproteina niske gusto&#x0107;e, triglicerida, kreatinina, glukoze u krvi i razine glikoziliranog hemoglobina (skupine RA i OA). (<xref ref-type="bibr" rid="r7"><italic>7</italic></xref>-<xref ref-type="bibr" rid="r9"><italic>9</italic></xref>) Oralni test optere&#x0107;enja glukozom tako&#x0111;er je proveden kada je bilo indicirano. Mjerene su razine RF i anti-CCP (samo zavr&#x0161;na vizita) kod ispitanika s RA. Sve obrasce ispunio je lije&#x010D;nik na temelju dobivenih informacija. Na zavr&#x0161;noj viziti upotrebljavani su obrasci (CRF) za zavr&#x0161;nu vizitu: upitnik o svim dijagnosticiranim kardiovaskularnim bolestima, &#x010D;imbenicima rizika za kardiovaskularne bolesti, bolestima koje nisu povezane s kardiovaskularnim bolestima, kori&#x0161;tenim lijekovima i kiru&#x0161;kim zahvatima koje su ispitanici imali tijekom razdoblja istra&#x017E;ivanja. Detaljni pregledi i op&#x0161;irne laboratorijske pretrage u&#x010D;injeni u inicijalnoj (uklju&#x010D;noj) viziti i u zavr&#x0161;noj viziti.</p>
<p>Za ispitanike koji su umrli ili postali nedostupni tijekom razdoblja ispitivanja napravljen je poseban obrazac; ako je ispitanik preminuo, zabilje&#x017E;en je uzrok i vrijeme smrti te podatci o poznatim &#x010D;imbenicima rizika za KVB.</p>
<sec>
<title>Statisti&#x010D;ke metode</title>
<p>Deskriptivna statistika kori&#x0161;tena je za opisivanje i sa&#x017E;imanje podataka studije. Za testiranje na&#x0161;ih hipoteza primijenjena je inferencijalna statistika. Za analizu odnosa izme&#x0111;u kvalitativnih varijabli kori&#x0161;ten je hi-kvadrat test neovisnosti. Fisherov egzaktni test kori&#x0161;ten je kada nisu ispunjene pretpostavke za hi-kvadrat test. Razina zna&#x010D;ajnosti postavljena je tako da se p &lt; 0,05 smatra statisti&#x010D;ki zna&#x010D;ajnim.</p>
<sec>
<title>Eti&#x010D;ki standardi:</title>
<p>Ovo ispitivanje provedeno je u skladu s Helsin&#x0161;kom deklaracijom, a odobreno od strane eti&#x010D;kih povjerenstava KBC-a Osijek i Medicinskog fakulteta Osijek (br. 602-04/21-08/07).</p>
</sec>
<sec>
<title>Informirani pristanak za sudjelovanje:</title>
<p>Pisani informirani pristanak dobiven je od svih sudionika.</p>
</sec>

<title>Rezultati</title>
<p>Od 201 ispitanika uklju&#x010D;enog u ovo ispitivanje izme&#x0111;u 2008./9. i 2016./17., 124 je imalo RA, a 77 ih je imalo OA. Ukupno 137 ispitanika (82 s RA i 55 s OA) zavr&#x0161;ilo je ispitivanje. Razlozi nezavr&#x0161;avanja ispitivanja bili su smrt (58 ispitanika; 41 s RA i 17 s OA), odbijanje nastavka ispitivanja ili preseljenje (6 ispitanika; 1 s RA i 5 s OA).</p>
<p>Ispitnu (RA) skupinu pri inicijalnoj viziti &#x010D;inilo je 25 mu&#x0161;karaca i 99 &#x017E;ena, dok je kontrolnu (OA) skupinu &#x010D;inilo 11 mu&#x0161;karaca i 66 &#x017E;ena. Prosje&#x010D;na dob ispitanika u skupini s RA bila je 59,78 (37 &#x2013; 81) godina, dok je ona u skupini s OA bila 64,23 (27 &#x2013; 80) godine. Od ispitanika s RA, 90,2% bilo je pozitivno na RF.</p>
<p>U zavr&#x0161;noj viziti RA grupa uklju&#x010D;ivala je 82 ispitanika, a OA grupa 55 ispitanika. Prosje&#x010D;na dob ispitanika u skupini s RA bila je 65,5 (45 &#x2013; 90) godina, dok je ona u skupini s OA bila 71,2 (36 &#x2013; 89) godina.</p>
<p>Pedeset i osam od 201 ispitanika uklju&#x010D;enih u ispitivanje umrlo je tijekom osmogodi&#x0161;njeg razdoblja pra&#x0107;enja. Od toga je 41 (9 mu&#x0161;karaca i 32 &#x017E;ene) bio u skupini s RA, dok je 17 (tri mu&#x0161;karca i 14 &#x017E;ena) bilo u skupini s OA. Prosje&#x010D;na dob umrlih ispitanika u trenutku smrti bila je 74,3 godine (71,6 godina za skupinu s RA i 76,9 godina za skupinu s OA), pri &#x010D;emu je kardiovaskularna bolest bila uzrok smrti za 39 od 58 umrlih (29 od 41 za skupinu s RA i 10 od 17 za skupinu s OA).</p>
<p>Tijekom cijelog ispitivanja bilo je konstantno vi&#x0161;e pu&#x0161;a&#x010D;a u skupini s RA nego u skupini s OA (<xref ref-type="table" rid="t1">Table 1</xref>).</p>

<p>Ni u jednoj skupini trendovi u prevalenciji pu&#x0161;enja nisu se zna&#x010D;ajno mijenjali tijekom istra&#x017E;ivanja (<xref ref-type="table" rid="t3">Table 3 i</xref> <xref ref-type="table" rid="t4">Table 4</xref>). U skupini s RA trenutni pu&#x0161;a&#x010D;i (p = 0,028) i osobe s povije&#x0161;&#x0107;u pu&#x0161;enja (p = 0,016) imale su zna&#x010D;ajno ve&#x0107;i rizik od KVB-a (<xref ref-type="table" rid="t5">Table 5</xref>).</p>

<title>Rasprava</title>
<p>Pu&#x0161;enje uzrokuje sustavno proupalno stanje utje&#x010D;u&#x0107;i na imunolo&#x0161;ki sustav, stani&#x010D;ne i humoralne komponente. (<xref ref-type="bibr" rid="r9"><italic>9</italic></xref>, <xref ref-type="bibr" rid="r10"><italic>10</italic></xref>) &#x010C;ini se da kroni&#x010D;no pu&#x0161;enje cigareta remeti stani&#x010D;nu regulatornu aktivnost, upalne odgovore i morfolo&#x0161;ke, fiziolo&#x0161;ke, biokemijske i enzimske odgovore svojim utjecajem na uro&#x0111;ene i adaptivne imunolo&#x0161;ke odgovore. (<xref ref-type="bibr" rid="r10"><italic>10</italic></xref>-<xref ref-type="bibr" rid="r12"><italic>12</italic></xref>) Prema Bracke i suradnicima, pu&#x0161;enje je pove&#x0107;alo ekspresiju mi&#x0161;jih plu&#x0107;nih makrofaga i enzima matri&#x010D;ne metaloproteinaze dendriti&#x010D;nih stanica (MMP)-12 (makrofagna elastaza). (<xref ref-type="bibr" rid="r13"><italic>13</italic></xref>) Dodatno je MMP-12 povezan s patofiziologijom RA. (<xref ref-type="bibr" rid="r14"><italic>14</italic></xref>, <xref ref-type="bibr" rid="r15"><italic>15</italic></xref>) Za razliku od osteoartritisa, sinovijalno tkivo kod RA imalo je vi&#x0161;e razine glasni&#x010D;ke RNA14 MMP-12 i prekomjernu ekspresiju MMP-12, kod transgenih kuni&#x0107;a zna&#x010D;ajno je pogor&#x0161;alo artriti&#x010D;ne lezije i dovelo do zna&#x010D;ajnog sinovijalnog zadebljanja, formiranja panusa, izra&#x017E;ene infiltracije makrofaga u ranoj fazi i zna&#x010D;ajnog o&#x0161;te&#x0107;enja zglobne hrskavice u kasnijoj fazi. (<xref ref-type="bibr" rid="r14"><italic>14</italic></xref>, <xref ref-type="bibr" rid="r15"><italic>15</italic></xref>) Tako&#x0111;er, studije su pokazale da su pu&#x0161;a&#x010D;i imali vi&#x0161;e razine proMMP-2 i proMMP-9 nego nepu&#x0161;a&#x010D;i. Tako&#x0111;er je sugerirano da MMP-9 proizveden od sinovijalnih fibroblasta RA mo&#x017E;e izravno doprinijeti o&#x0161;te&#x0107;enju zglobova kod RA. (<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>, <xref ref-type="bibr" rid="r17"><italic>17</italic></xref>)</p>
<p>Kroni&#x010D;na izlo&#x017E;enost dimu &#x010D;esto rezultira leukocitozom sa smanjenom funkcijom leukocita, (<xref ref-type="bibr" rid="r10"><italic>10</italic></xref>, <xref ref-type="bibr" rid="r18"><italic>18</italic></xref>, <xref ref-type="bibr" rid="r19"><italic>19</italic></xref>) a dugotrajno pu&#x0161;enje sni&#x017E;ava serumske imunoglobuline i razine odgovora specifi&#x010D;nih protutijela. (<xref ref-type="bibr" rid="r19"><italic>19</italic></xref>) Unato&#x010D; ovim otkri&#x0107;ima, vi&#x0161;e razine autoantitijela, osobito antinuklearnih i anti-RF, mogle bi objasniti za&#x0161;to postoji ve&#x0107;a vjerojatnost za razvoj autoimunosnih poreme&#x0107;aja poput RA. (<xref ref-type="bibr" rid="r11"><italic>11</italic></xref>)</p>
<p>Vi&#x0161;e razine fibrinogena, CRP-a, ICAM-1, E-selektina i proupalnih citokina (npr. TNF-, IL-1, IL-5, IL-6, IL-8, IL-13) u pu&#x0161;a&#x010D;a, &#x0161;to je u korelaciji s trenutnom i pro&#x0161;lom izlo&#x017E;enosti pu&#x0161;enju, dokaz su indukcije upalnog odgovora. (<xref ref-type="bibr" rid="r20"><italic>20</italic></xref>-<xref ref-type="bibr" rid="r25"><italic>25</italic></xref>) TNF-, IL-1 i IL-6 klju&#x010D;ni su u etiopatogenezi RA, ali i KVB-a. (<xref ref-type="bibr" rid="r25"><italic>25</italic></xref>, <xref ref-type="bibr" rid="r26"><italic>26</italic></xref>)</p>
<p>Autoantitijela protiv citruliniranih proteina mogu potaknuti autoimunosni odgovor i mogu se na&#x0107;i u mnogih pacijenata s RA. (<xref ref-type="bibr" rid="r27"><italic>27</italic></xref>) Poznato je da pu&#x0161;enje uzrokuje imunosne odgovore koji su HLA-DR-ograni&#x010D;eni na autoantigene modificirane citrulinacijom. Pu&#x0161;a&#x010D;i su jasno izlo&#x017E;eni ve&#x0107;em riziku od stvaranja anti-CCP protutijela, a kod genetski predisponiranih osoba sa zajedni&#x010D;kim epitopom HLA-DRB1 gena, &#x010D;ini se da je ovaj rizik u korelaciji s te&#x017E;inom stanja. (<xref ref-type="bibr" rid="r28"><italic>28</italic></xref>-<xref ref-type="bibr" rid="r30"><italic>30</italic></xref>)</p>
<p>Pu&#x0161;enje cigareta zna&#x010D;ajan je &#x010D;imbenik rizika za KVB. Aktivni pu&#x0161;a&#x010D;i i pasivni pu&#x0161;a&#x010D;i imaju 50%, odnosno 30% &#x0161;anse da &#x0107;e umrijeti zbog pu&#x0161;enja. Prosje&#x010D;ni &#x017E;ivotni vijek pu&#x0161;a&#x010D;a skra&#x0107;uje se za otprilike deset godina. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>) Pu&#x0161;enje u kombinaciji s RA pove&#x0107;ava rizik od KVB-a za otprilike 25 &#x2013; 50%. (<xref ref-type="bibr" rid="r31"><italic>31</italic></xref>, <xref ref-type="bibr" rid="r32"><italic>32</italic></xref>) U ovom je ispitivanju prevalencija pu&#x0161;enja u skupini s RA (ispitna) bila zna&#x010D;ajno vi&#x0161;a nego u skupini s OA (kontrolna) i op&#x0107;oj populaciji Hrvatske, koja je jedna od najvi&#x0161;ih u Europi (31,1%). (<xref ref-type="bibr" rid="r33"><italic>33</italic></xref>) Osim toga, prevalencija pu&#x0161;enja cigareta me&#x0111;u umrlima u skupini s RA bila je vi&#x0161;a nego me&#x0111;u umrlima u skupini s OA, kao i kod svih umrlih i &#x017E;ivih sudionika s KVB-om ukupno.</p>
<p>Trendovi u prevalenciji pu&#x0161;enja cigareta u ispitivanoj i kontrolnim skupinama nisu se zna&#x010D;ajno mijenjali tijekom ispitivanja. Ovo sugerira da pu&#x0161;a&#x010D;i imaju pove&#x0107;an rizik od RA. Trend kontinuiranog pu&#x0161;enja u bolesnika s RA mo&#x017E;e se objasniti poku&#x0161;ajima smanjenja stresa uzrokovanog simptomima RA. Pu&#x0161;enje cigareta va&#x017E;an je promjenjivi &#x010D;imbenik rizika za RA. Korelacija izme&#x0111;u pu&#x0161;enja cigareta i RA dokazana je u epidemiolo&#x0161;kim, <italic>in vivo</italic> i studijama na &#x017E;ivotinjama. (<xref ref-type="bibr" rid="r34"><italic>34</italic></xref>) Pu&#x0161;enje pove&#x0107;ava rizik od razvoja RA za otprilike 40 &#x2013; 50% s odre&#x0111;enim u&#x010D;inkom ovisnim o dozi, &#x0161;to je u skladu s na&#x0161;im rezultatima. (<xref ref-type="bibr" rid="r34"><italic>34</italic></xref>-<xref ref-type="bibr" rid="r36"><italic>36</italic></xref>) Prethodne studije su pokazale da i sada&#x0161;nji i biv&#x0161;i pu&#x0161;a&#x010D;i s RA imaju te&#x017E;e simptome, te&#x017E;a o&#x0161;te&#x0107;enja zglobova i slabiji odgovor na terapiju od oni koji nikad nisu pu&#x0161;ili. (<xref ref-type="bibr" rid="r36"><italic>36</italic></xref>-<xref ref-type="bibr" rid="r38"><italic>38</italic></xref>) Me&#x0111;utim, razlog u pozadini trenda kontinuiranog pu&#x0161;enja cigareta kod bolesnika s RA ostaje nejasan. Me&#x0111;u pu&#x0161;a&#x010D;ima u skupini RA u ovom ispitivanju, sada&#x0161;nji i biv&#x0161;i pu&#x0161;a&#x010D;i, ispitanici koji su zavr&#x0161;ili ispitivanje i umrli ispitanici pokazali su zna&#x010D;ajno pove&#x0107;anu incidenciju KVB-a, &#x0161;to je u skladu s postoje&#x0107;im spoznajama o povezanosti pu&#x0161;enja i KVB-a. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>)</p>
<sec>
<title>Ograni&#x010D;enja</title>
<p>Ovo ispitivanje ima neka ograni&#x010D;enja. Prvo, podatci o pu&#x0161;a&#x010D;kim navikama dobiveni su pomo&#x0107;u upitnika za samoprocjenu koje su istra&#x017E;iva&#x010D;i ispunili informacijama dobivenim od ispitanika; stoga se dani odgovori nisu mogli potvrditi. Drugo, podatci za preminule dobiveni su iz kartona mrtvozornika, koji nisu napisali lije&#x010D;nici. Me&#x0111;utim, te podatke koristi Nacionalni institut za zdravstvo i smatraju se va&#x017E;e&#x0107;ima za slu&#x017E;bena statisti&#x010D;ka izvje&#x0161;&#x0107;a; posebno, statisti&#x010D;ke analize mortaliteta slu&#x017E;e samo za prikazivanje veli&#x010D;ine problema, jer je uzorak bio premalen i relevantne statisti&#x010D;ke analize nisu se mogle provesti.</p>

</sec>
<sec sec-type="other4">
<title>Zaklju&#x010D;ci</title>
<p>Provedeno je samo nekoliko studija kako bi se istra&#x017E;ila korelacija izme&#x0111;u pu&#x0161;enja, RA i KVB-a. Pu&#x0161;enje i dalje predstavlja najve&#x0107;i javnozdravstveni problem u svijetu, posebice u isto&#x010D;noj Europi gdje je prevalencija pu&#x0161;a&#x010D;a me&#x0111;u najvi&#x0161;ima u Europi. Stoga su potrebna daljnja istra&#x017E;ivanja kako bi se razjasnila povezanost izme&#x0111;u RA i pu&#x0161;enja. &#x0160;tovi&#x0161;e, lije&#x010D;nici bi trebali nastojati poticati i educirati pacijente s RA o va&#x017E;nosti prestanka pu&#x0161;enja.</p>
