<title>SA&#x017D;ETAK</title>
<sec><title>Pozadina</title><p>Diljem svijeta op&#x0107;e je prihva&#x0107;ena &#x010D;injenica da bi redovita organizacija i obvezatnost poha&#x0111;anja te&#x010D;ajeva kardiopulmonalne reanimacije (KPR) zdravstvenih djelatnika na svim razinama zdravstvenog sustava trebala biti uobi&#x010D;ajena praksa. U Republici Hrvatskoj je redovitost poha&#x0111;anja te&#x010D;aja proizvoljna, a tek mali broj zdravstvenih ustanova redovito organizira te&#x010D;ajeve svojim djelatnicima. Od 2015. godine kardiolo&#x0161;ki tim tada&#x0161;njeg Doma zdravlja Osijek, sada&#x0161;njeg Doma zdravlja Osje&#x010D;ko-baranjske &#x017E;upanije, prosje&#x010D;no je svake dvije godine organizirao te&#x010D;ajeve KPR-a i hitnih stanja namijenjene zdravstvenim djelatnicima ustanove. Od 2019. godine sastavni dio te&#x010D;aja je pismeni ispit.</p></sec>
<sec><title>Ciljevi</title><p>primarni &#x2013; procijeniti znanje djelatnika primarne zdravstvene za&#x0161;tite (PZZ) o pravilnom postupku KPR-a te usporediti to&#x010D;nost odgovora prije, neposredno i tri godine nakon provedenog te&#x010D;aja. Sekundarni ciljevi bili su utvrditi postotak zdravstvenih djelatnika koji se odaziva te&#x010D;aju i procijeniti utjecaj pandemije COVID-19.</p></sec>
<sec><title>Sudionici i metode</title><p>2019. godine odazvalo se 87 od 258 (34%) zdravstvenih djelatnika tada&#x0161;njeg Doma zdravlja Osijek. Na novi te&#x010D;aj organiziran 2022. godine prijavila su se 133 od 572 (23%) zdravstvena djelatnika Doma zdravlja Osje&#x010D;ko-baranjske &#x017E;upanije, me&#x0111;u kojima je bilo 26 (19%) ponavlja&#x010D;a. Usporedili smo odgovore na pitanja prije oba te&#x010D;aja i neposredno nakon te&#x010D;aja organiziranog 2019. godine.</p></sec>
<sec><title>Rezultati</title><p>Na predispitu 2019. godine prosje&#x010D;an postotak to&#x010D;nih odgovora bio je 58%, dok je na zavr&#x0161;nom ispitu iznosio 83%. Prosje&#x010D;an postotak to&#x010D;nih odgovora na predispitu 2022. godine bio je 66%, dok su ponavlja&#x010D;i ostvarili prosje&#x010D;no 75% to&#x010D;nih odgovora. Godine 2022. 7% zdravstvenih djelatnika bilo je neodlu&#x010D;no oko pravovremenog zapo&#x010D;injanja postupka KPR-a uslijed straha od infekcije virusom SARS-CoV-2.</p></sec>
<sec><title>Zaklju&#x010D;ak</title><p>Poznavanje pravilnog postupka KPR-a u zdravstvenih djelatnika s vremenom slabi, a redovitom reedukacijom smanjuje se fluktuacija znanja. Tri godine nakon provedene edukacije ponavlja&#x010D;i ostvaruju bolji prosjek to&#x010D;nih odgovora na inicijalnom testu. Te&#x010D;ajevima se odaziva svega 25 &#x2013; 30% zdravstvenih djelatnika, a manji postotak djelatnika redovno ih ponavlja. COVID-19 je negativno utjecao na odlu&#x010D;nost u pravovremenom pristupanju KPR-u.</p></sec>

<title>SUMMARY</title>
<sec><title>Background</title><p>Regular organization and mandatory attendance of cardiopulmonary resuscitation (CPR) courses for healthcare professionals (HCP) at all levels of the healthcare system should be a common practice. In the Republic of Croatia, the regularity of course attendance is arbitrary, and only a small number of health institutions organize courses for their employees regularly. Since 2015, the cardiology team of the former Health Center Osijek, currently the Health Center of Osijek-Baranja County, has organized CPR and emergency courses for HCP of the institution every two years. Since 2019, an integral part of the course has been a written exam.</p></sec>
<sec><title>Aims</title><p>Primary aims were to assess the knowledge of primary HCP about the correct CPR procedure and to compare the accuracy of answers before, immediately and three years after the course. Secondary aims were to determine the percentage of HCP responding to the courses and to assess the impact of the COVID-19 pandemic.</p></sec>
<sec><title>Participants and methods</title><p>In 2019, 87 out of 258 (34%) HCP of the former Osijek Health Center responded. For the new course organized in 2022, 133 out of 572 (23%) HCP of the Health Center of Osijek-Baranja County applied, among whom were 26 (19%) repeaters. We compared the answers to the questions before both courses and immediately after the course organized in 2019. <italic>Results:</italic> In the 2019 preliminary exam the average percentage of correct answers was 58%, while in the final exam it was 83%. The average percentage of correct answers in the 2022 preliminary exam was 66%, while repeaters achieved an average of 75% correct answers. In 2022, 7% of HCP were uncertain about the timely initiation of CPR due to the fear of infection with the SARS-CoV 2 virus.</p></sec>
<sec><title>Conclusion</title><p>The knowledge of the correct CPR procedure among HCP weakens over time, and regular re-education reduces the fluctuation of knowledge. Three years after the course repeaters achieve a better average of correct answers on the initial test. Only 25&#x2013;30% of HCP attend the courses, and a smaller percentage of them repeat it regularly. COVID-19 has had a negative impact on the determination to access CPR in a timely manner.</p></sec>

<p>Iznenadna sr&#x010D;ana smrt je iznenadni, neo&#x010D;ekivani prestanak rada srca, tzv. sr&#x010D;ani arest (CA, od engl. <italic>cardiac arrest</italic>), sa smrtnim ishodom unutar jednog sata od pojave simptoma. Neovisno o uzroku CA, najva&#x017E;nije intervencije su univerzalne te uklju&#x010D;uju: rano prepoznavanje i pozivanje pomo&#x0107;i, zbrinjavanje bolesnika, brzu defibrilaciju i kvalitetnu kardiopulmonalnu reanimaciju (KPR) s minimalnim prekidom kompresije prsnog ko&#x0161;a, lije&#x010D;enjem reverzibilnih uzroka i njegom nakon reanimacije. Ne samo &#x017E;ivot, ve&#x0107; i budu&#x0107;a kvaliteta &#x017E;ivota (QoL, od engl. <italic>quality of life</italic>) osobe kod koje se uspije posti&#x0107;i povratak spontane cirkulacije (ROSC, od engl. <italic>return of spontaneous circulation</italic>) ovisi o vremenskom razdoblju izme&#x0111;u pojave CA i zapo&#x010D;injanja kvalitetnog postupka KPR-a. &#x0160;to je bolje znanje i br&#x017E;a reakcija svjedoka, ve&#x0107;e su &#x0161;anse za pre&#x017E;ivljavanje uz bolju QoL &#x017E;rtve. Upravo je to jedan od razloga velikih razlika u pre&#x017E;ivljenju izme&#x0111;u izvanbolni&#x010D;kog (OHCA, od engl. <italic>out-of-hospital cardiac arrest</italic>) i bolni&#x010D;kog sr&#x010D;anog aresta (IHCA, od engl. <italic>in-hospital cardiac arrest</italic>).</p>
<p>Prema podatcima Europskog vije&#x0107;a za reanimaciju (ERC, od engl. <italic>European Resuscitation Council</italic>) iz 2021. godine (<xref ref-type="bibr" rid="r1"><italic>1</italic></xref>), incidencija OHCA u Europskim zemljama iznosila je izme&#x0111;u 67 i 170 na 100.000 stanovnika, neodgo&#x0111;ena KPR (od strane svjedoka) pru&#x017E;ena je u prosje&#x010D;no 58% slu&#x010D;ajeva, automatski vanjski defibrilator (AED, od engl. <italic>automated external defibrillators</italic>) kori&#x0161;ten je u 28% slu&#x010D;ajeva, a stopa pre&#x017E;ivljenja je iznosila prosje&#x010D;no 8%. Pritom postoje velike geografske razlike u pre&#x017E;ivljenju OHCA unutar zemalja Europske unije (<xref ref-type="bibr" rid="r2"><italic>2</italic></xref>) koje su uvjetovane: sociodemografskim karakteristikama stanovni&#x0161;tva, varijabilnostima u organizaciji hitne medicinske pomo&#x0107;i (HMP), kori&#x0161;tenjem dispe&#x010D;erskih shema pru&#x017E;anja hitne pomo&#x0107;i (<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>), ali i razli&#x010D;itom razinom i kvalitetom obrazovanja i osposobljenosti osoba koje pru&#x017E;aju prvu pomo&#x0107;. Europska dr&#x017E;ava s najve&#x0107;om stopom pre&#x017E;ivljenja OHCA, &#x010D;ak 25%, jest Norve&#x0161;ka u kojoj je te&#x010D;aj KPR-a sastavni dio nacionalnoga &#x0161;kolskog kurikuluma jo&#x0161; od davne 1961. godine. (<xref ref-type="bibr" rid="r4"><italic>4</italic></xref>, <xref ref-type="bibr" rid="r5"><italic>5</italic></xref>)</p>
<p>Op&#x0107;enito se smatra da bi svi zdravstveni djelatnici trebali znati pru&#x017E;iti visokokvalitetnu KPR bilo kada i bilo gdje. Problem le&#x017E;i u &#x010D;injenici &#x0161;to ve&#x0107;ina zdravstvenih djelatnika prolazi osnovnu obuku iz KPR-a u srednjo&#x0161;kolskim i/ili studentskim danima, a nakon toga mali broj njih ima priliku aktivno sudjelovati u pru&#x017E;anju KPR-a. Ovaj problem je dobro poznat u cijelom svijetu, zbog &#x010D;ega sve renomirane svjetske organizacije koje se bave problemom OHCA, poput Me&#x0111;unarodnog odbora za reanimaciju (ILCOR, od engl. <italic>International Liaison Committee on Resuscitation</italic>), Ameri&#x010D;kog sr&#x010D;anog udru&#x017E;enja (AHA, od engl. <italic>American Heart Association</italic>) i ERC-a, sna&#x017E;no preporu&#x010D;uju reedukaciju i obuku zdravstvenih djelatnika. (<xref ref-type="bibr" rid="r6"><italic>6</italic></xref>&#x2013;<xref ref-type="bibr" rid="r9"><italic>9</italic></xref>) Svi se oni sla&#x017E;u da bi zdravstveni djelatnici na svim razinama zdravstvenog sustava trebali redovito poha&#x0111;ati reedukaciju prosje&#x010D;no svake dvije godine, a neki i &#x010D;e&#x0161;&#x0107;e. Preporu&#x010D;uje se da se prilikom te&#x010D;ajeva KPR-a u&#x010D;enje <italic>ex cathedra</italic> kombinira s virtualnim scenarijima i prakti&#x010D;nim vje&#x017E;bama u kojima se omogu&#x0107;uje aktivna interakcija svih sudionika. U skladu s navedenim, u ve&#x0107;ini razvijenih zemalja svijeta periodi&#x010D;ne reedukacije zdravstvenih djelatnika postupcima KPR-a obvezni su dio cjelo&#x017E;ivotnog u&#x010D;enja. Ipak, to ne vrijedi za zdravstvene djelatnike u Republici Hrvatskoj, gdje je odluka o potrebi reedukacije ve&#x0107;inom proizvoljna.</p>
<p>U razdoblju od 2015. do 2022. godine kardiolo&#x0161;ki tim nekada&#x0161;njeg Doma zdravlja Osijek, a sada&#x0161;njeg Doma zdravlja Osje&#x010D;ko-baranjske &#x017E;upanije, u suradnji s ravnateljima i lije&#x010D;nicima PZZ-a, kolegama kardiolozima i internistima Klini&#x010D;koga bolni&#x010D;kog centra Osijek, pet puta je organizirao te&#x010D;ajeve KPR-a za zdravstvene djelatnike ustanove koji su se dobrovoljno prijavljivali. Te&#x010D;ajevi su se sastojali od pet sati predavanja na temu hitnih stanja, pravilnog postupka KPR-a i interpretacije elektrokardiograma (EKG) te tri sata prakti&#x010D;nih vje&#x017E;bi na lutkama uz kori&#x0161;tenje AED-a. Tijekom prakti&#x010D;nog dijela zdravstveni djelatnici su bili podijeljeni u grupe po deset sudionika. Svaka grupa je aktivno sudjelovala u tri zasebne vje&#x017E;be koje su prethodno osmi&#x0161;ljene kao scenariji za poticanje timskog rada, prepoznavanja i rje&#x0161;avanja hitnih stanja te vje&#x017E;bi interpretacije EKG-a. Svaku vje&#x017E;bu su provodila dva lije&#x010D;nika uz asistenciju jedne medicinske sestre. Svi sudionici su trebali prilikom izvo&#x0111;enja vje&#x017E;bi nekoliko puta ponoviti vanjsku masa&#x017E;u srca i ventilaciju usta na usta, usta na nos te ventilaciju uz pomo&#x0107; orofaringealnog tubusa i ambu maske. Godine 2022. te&#x010D;aj smo provodili u skladu s tada&#x0161;njim znanstvenim saznanjima i slu&#x017E;benim uputama za provo&#x0111;enje KPR-a u doba pandemije COVID-19 (<xref ref-type="bibr" rid="r10"><italic>10</italic></xref>, <xref ref-type="bibr" rid="r11"><italic>11</italic></xref>) izbjegavaju&#x0107;i pritom direktnu ventilaciju usta na usta i usta na nos. Od 2019. godine sastavni je dio te&#x010D;aja i pismeni ispit koji su sudionici ispunjavali neposredno prije i nakon te&#x010D;aja. Za potrebe samog te&#x010D;aja napisan je i priru&#x010D;nik o KPR-u i poznavanju hitnih stanja koja joj prethode, koji je nekoliko dana prije po&#x010D;etka te&#x010D;aja putem elektronske po&#x0161;te proslije&#x0111;en svim prijavljenim zdravstvenim djelatnicima, a sadr&#x017E;ajem je pratio ne samo va&#x017E;e&#x0107;e smjernice AHA i ERC vezane za postupak KPR-a (<xref ref-type="bibr" rid="r7"><italic>7</italic></xref>, <xref ref-type="bibr" rid="r8"><italic>8</italic></xref>, <xref ref-type="bibr" rid="r12"><italic>12</italic></xref>&#x2013;<xref ref-type="bibr" rid="r16"><italic>16</italic></xref>), nego i pravilno lije&#x010D;enje akutnih komplikacija &#x0161;e&#x0107;erne bolesti (<xref ref-type="bibr" rid="r17"><italic>17</italic></xref>), epilepti&#x010D;kog napada, opstrukcije di&#x0161;nih puteva i alergijskih reakcija (<xref ref-type="bibr" rid="r18"><italic>18</italic></xref>). Oba puta te&#x010D;aj je bio organiziran i prijavljen kao skup bez kotizacije i kao takav bodovan od Hrvatske lije&#x010D;ni&#x010D;ke komore, Hrvatske komore medicinskih sestara i Hrvatske komore fizioterapeuta.</p>
<p>Na temelju prikupljenih anonimnih podataka proveli smo retrospektivno istra&#x017E;ivanje kojim smo htjeli procijeniti znanje djelatnika primarne zdravstvene za&#x0161;tite (PZZ) o pravilnom postupku KPR-a te usporediti to&#x010D;nost odgovora prije, neposredno i tri godine nakon provedenog te&#x010D;aja. Sekundarni ciljevi bili su utvrditi postotak zdravstvenih djelatnika koji se odazivaju te&#x010D;aju te procijeniti utjecaj pandemije COVID-19.</p>

<title>Sudionici i metode</title>
<p>Obavijest o organizaciji te&#x010D;aja KPR-a poslana je elektroni&#x010D;kom po&#x0161;tom svim zdravstvenim djelatnicima na&#x0161;e ustanove nekoliko mjeseci prije planiranog po&#x010D;etka te&#x010D;aja i uklju&#x010D;ivala je poveznicu za <italic>online</italic> prijavu. Obvezni dio prijave bili su anonimna anketa i ispit. Anketa je sadr&#x017E;avala pitanja koja su uklju&#x010D;ivala zanimanje, dob, radno iskustvo, dosada&#x0161;nje obrazovanje, zadovoljstvo dosada&#x0161;njim poznavanjem postupka KPR-a, podatak o prethodnom sudjelovanju na te&#x010D;ajevima KPR-a, poznavanje adekvatne primjene AED-a te spremnost neodgo&#x0111;enog pru&#x017E;anja prve pomo&#x0107;i i zapo&#x010D;injanja KPR-a uz upotrebu AED-a unutar i izvan zdravstvene ustanove. Prije te&#x010D;aja 2019. godine ispit je sadr&#x017E;avao samo dvanaest tzv. &#x201E;kontrolnih&#x201C; pitanja, dok su zavr&#x0161;ni ispit iste godine i prijavni ispit 2022. godine obuhva&#x0107;ali ukupno 30 pitanja. Pitanja su sastavili &#x010D;lanovi kardiolo&#x0161;kog tima Doma zdravlja Osje&#x010D;ko-baranjske &#x017E;upanije, a na svako pitanje bila su ponu&#x0111;ena &#x010D;etiri odgovora od kojih je samo jedan bio to&#x010D;an (Dodatak 1). Ispit smo smatrali pozitivno rije&#x0161;enim ako je ostvareno &#x2265;20 (66%) to&#x010D;nih odgovora. Na kraju te&#x010D;aja organiziranog 2019. godine, osim zavr&#x0161;nog ispita, sudionici su odgovarali i na zavr&#x0161;nu anonimnu anketu u kojoj su ocjenjivali predavanja i prakti&#x010D;ni dio te&#x010D;aja i mogli su izraziti svoje stavove, zadovoljstvo te&#x010D;ajem i nau&#x010D;enim gradivom te dati prijedloge za budu&#x0107;e te&#x010D;ajeve.</p>
<p>Na oba te&#x010D;aja sudjelovala su ukupno 194 zdravstvena djelatnika, me&#x0111;u kojima njih 26 na oba. Radi se o retrospektivnom istra&#x017E;ivanju koje je provedeno u skladu s eti&#x010D;kim na&#x010D;elima Deklaracije iz Helsinkija iz 2000. godine i dopunama iz 2002. i 2004. godine. Pristup prikupljenim podatcima imali su isklju&#x010D;ivo &#x010D;lanovi istra&#x017E;iva&#x010D;kog tima. Budu&#x0107;i da se radilo o retrospektivnom istra&#x017E;ivanju provedenom na anonimnim podatcima nije tra&#x017E;ena suglasnost Eti&#x010D;kog povjerenstva ustanove.</p>

<sec sec-type="other2">
<title>Statisti&#x010D;ka obrada</title>
<p>Za statisti&#x010D;ku analizu podataka kori&#x0161;ten je statisti&#x010D;ki program <italic>MedCalc&#x00AE; Statistical Software</italic> verzija 20.215 (MedCalc Software Ltd., Ostend, Belgium; <ext-link ext-link-type="uri" xlink:href="https://www.medcalc.org">https://www.medcalc.org</ext-link>). Razlike kategorijskih varijabli testirane su hi-kvadrat (&#x03C7;2) testom. Za testiranje razlika numeri&#x010D;kih varijabli izme&#x0111;u dvije nezavisne skupine koristili smo Mann-Whitneyev U-test, a izme&#x0111;u tri i vi&#x0161;e nezavisnih skupina Kruskal-Wallisov test. Za testiranje razlika izme&#x0111;u dva mjerenja (po&#x010D;etno i zavr&#x0161;no) koristili smo Wilcoxonov test. Razina statisti&#x010D;ke zna&#x010D;ajnosti postavljena je na p &lt;0,05.</p>
</sec>
<sec sec-type="other3">
<title>Rezultati</title>
<p>Godine 2019. te&#x010D;aju se odazvalo 87/258 (34%) zdravstvenih djelatnika tada&#x0161;njeg Doma zdravlja Osijek. Na te&#x010D;aj organiziran 2022. godine prijavila su se 133/572 (23%) zdravstvena djelatnika Doma zdravlja Osje&#x010D;ko-baranjske &#x017E;upanije.</p>
<p>U <xref ref-type="table" rid="t1">Table 1</xref> prikazane su op&#x0107;e karakteristike sudionika obaju te&#x010D;aja. 40 &#x2013; 50% sudionika po zanimanju su bili lije&#x010D;nici ili stomatolozi, a me&#x0111;u ostalim zdravstvenim djelatnicima bile su medicinske sestre/tehni&#x010D;ari, dentalni asistenti, fizioterapeuti i medicinski laboranti. Najve&#x0107;e razlike me&#x0111;u sudionicima obaju te&#x010D;aja bile su u postotku sudionika koji nisu poha&#x0111;ali te&#x010D;aj KPR-a nakon &#x0161;kolovanja &#x2013; 2019. godine bilo ih je svega 3%, a 2022. godine &#x010D;ak 47%.</p>

<p>Po pitanju poznavanja pravilnog kori&#x0161;tenja AED-a, prije te&#x010D;aja 2019. godine pozitivan stav je imao 51 (59%), a 2022. godine 70 (53%) sudionika.</p>
<p>99% sudionika obaju te&#x010D;aja imalo je pozitivan stav glede du&#x017E;nosti pru&#x017E;anja HMP-a i izvan zdravstvene ustanove u kojoj rade. Godine 2022. njih 10 (7%) je bilo nesigurno oko samog postupka i pravovremenog zapo&#x010D;injanja KPR-a uslijed straha od infekcije virusom SARS-CoV-2.</p>
<p>U <xref ref-type="table" rid="t2">Table 2</xref> prikazali smo broj to&#x010D;nih odgovora na kontrolna pitanja prije i nakon te&#x010D;aja, te broj to&#x010D;nih odgovora na zavr&#x0161;nom ispitu te&#x010D;aja organiziranog 2019. godine. Isto smo usporedili s osnovnim karakteristikama sudionika. Iz navedene tablice mo&#x017E;e se uo&#x010D;iti da je prosje&#x010D;an broj to&#x010D;nih odgovora na kontrolna pitanja, neovisno o osnovnim karakteristikama sudionika, prije te&#x010D;aja bio 7/12 (58%), a nakon te&#x010D;aja 10/12 (83%). Na osnovi tih rezultata mo&#x017E;emo zaklju&#x010D;iti da su svi sudionici postigli signifikantno pove&#x0107;anje broja to&#x010D;nih odgovora nakon provedenog te&#x010D;aja.</p>

<p>Prosje&#x010D;an broj to&#x010D;nih odgovora na zavr&#x0161;nom ispitu 2019. godine bio je 26/30 (86%). Test je pozitivno rije&#x0161;ilo 77 sudionika (89%). Zdravstveni djelatnici s najkra&#x0107;im, kao i oni s 15 &#x2013; 30 godina radnog sta&#x017E;a, postigli su prosje&#x010D;no najmanji broj to&#x010D;nih odgovora. U pogledu prethodnog rada u bolnici, zadovoljstva edukacijom o KPR-u tijekom &#x0161;kolovanja, aktivnog sudjelovanja u reanimaciji i prethodnog poha&#x0111;anja te&#x010D;aja KPR-a nije bilo statisti&#x010D;ki zna&#x010D;ajne razlike u broju to&#x010D;nih odgovora me&#x0111;u sudionicima.</p>
<p>Prema rezultatima zavr&#x0161;ne ankete te&#x010D;aja organiziranog 2019. godine prosje&#x010D;na ocjena predavanja iznosila je 4,88, prakti&#x010D;nog dijela 4,59, te&#x010D;aj je ostvario o&#x010D;ekivanja kod 96% sudionika, 99% ih se smatralo sigurnijim u postupak KPR-a, a 95% ih se smatralo dovoljno educiranim za adekvatnu primjenu AED-a.</p>
<p>U <xref ref-type="table" rid="t3">Table 3</xref> prikazali smo rezultate predispita sudionika te&#x010D;aja KPR-a organiziranog 2022. godine. Iz tablice se mo&#x017E;e uo&#x010D;iti da su lije&#x010D;nici i stomatolozi ostvarili bolje rezultate u odnosu na ostale zdravstvene djelatnike, dok u pogledu drugih osnovnih karakteristika sudionika nije postojala statisti&#x010D;ki zna&#x010D;ajna razlika u broju ostvarenih bodova. Pritom je prosje&#x010D;an broj to&#x010D;nih odgovora svih sudionika na kontrolna pitanja bio 8/12 (66%), a na ukupna pitanja 22/30 (73%). Test je pozitivno rije&#x0161;ilo 106 (79%) sudionika.</p>

<p>U <xref ref-type="table" rid="t4">Table 4</xref> usporedili smo broj ostvarenih bodova na zavr&#x0161;nom ispitu 2019. godine i predispitu 2022. godine kod sudionika koji su sudjelovali na oba te&#x010D;aja. Rezultati su pokazali statisti&#x010D;ki zna&#x010D;ajan pad u broju to&#x010D;nih odgovora u odnosu na zavr&#x0161;ni ispit 2019. godine. Najve&#x0107;i pad je zabilje&#x017E;en u dobnoj skupini 20 &#x2013; 40 godina, kod onih s 5 &#x2013; 15 godina radnog sta&#x017E;a, koji su prethodno radili u bolnici, nisu bili zadovoljni edukacijom o KPR-u kroz &#x0161;kolovanje i nisu aktivno sudjelovali u reanimaciji. Sudionici kod kojih je od zadnjeg te&#x010D;aja KPR-a pro&#x0161;lo vi&#x0161;e od dvije godine imali su najve&#x0107;i pad to&#x010D;nih odgovora &#x2013; statisti&#x010D;ki zna&#x010D;ajniji na kontrolna pitanja (p = 0,003) u odnosu na ukupna pitanja (p = 0,05).</p>

<p>Uspore&#x0111;uju&#x0107;i te rezultate u odnosu na cijelu skupinu sudionika te&#x010D;aja 2022. godine, ponavlja&#x010D;i su imali prosje&#x010D;no jedan to&#x010D;an odgovor (8%) vi&#x0161;e u kontrolnim pitanjima, a tri to&#x010D;na odgovora (10%) vi&#x0161;e u ukupnim pitanjima.</p>
</sec>
<sec sec-type="other4">
<title>Rasprava</title>
<p>OHCA je najve&#x0107;i uzrok preuranjene smrti diljem svijeta, a najbolja preventivna mjera redukcije broja smrtnih slu&#x010D;ajeva jest podu&#x010D;avanje velikog broja gra&#x0111;ana osnovama KPR-a. (<xref ref-type="bibr" rid="r6"><italic>6</italic></xref>, <xref ref-type="bibr" rid="r19"><italic>19</italic></xref>) Pritom bi zdravstveni djelatnici trebali biti u mogu&#x0107;nosti pru&#x017E;iti kvalitetnu KPR u svako vrijeme i na svakom mjestu, a za to su potrebne redovite reedukacije. (<xref ref-type="bibr" rid="r7"><italic>7</italic></xref>&#x2013;<xref ref-type="bibr" rid="r9"><italic>9</italic></xref>)</p>
<p>Kako bi se utvrdio najbolji na&#x010D;in provo&#x0111;enja edukacija te vremenski interval u kojem se zadr&#x017E;ava zadovoljavaju&#x0107;e znanje, diljem svijeta su se provodila istra&#x017E;ivanja u svrhu utvr&#x0111;ivanja poznavanja pravilnog postupka i stavova zdravstvenih djelatnika u sigurnost provo&#x0111;enja KPR-a prije i nakon organiziranih te&#x010D;ajeva. (<xref ref-type="bibr" rid="r20"><italic>20</italic></xref>&#x2013;<xref ref-type="bibr" rid="r23"><italic>23</italic></xref>) Prema na&#x0161;im saznanjima u Republici Hrvatskoj do sada nije objavljen rad na tu temu.</p>
<p>Godine 2014. M&#x00E4;kinen i suradnici (<xref ref-type="bibr" rid="r20"><italic>20</italic></xref>), na osnovi istra&#x017E;ivanja provedenog tijekom 2008. godine me&#x0111;u zdravstvenim djelatnicima bolnica u &#x0160;vedskoj i Finskoj, objavili su kako velik postotak zdravstvenih djelatnika oklijeva zapo&#x010D;eti KPR uslijed manjka samopouzdanja, tjeskobe i straha da &#x0107;e na&#x0161;tetiti unesre&#x0107;enom. Takvi stavovi mogu rezultirati nepotrebnim gubitcima &#x017E;ivota, a redovitim te&#x010D;ajevima mo&#x017E;e se posti&#x0107;i bolje samopouzdanje. Upravo smo to potvrdili i u na&#x0161;em istra&#x017E;ivanju, &#x0161;to se sla&#x017E;e i s rezultatima drugih istra&#x017E;iva&#x010D;a. (<xref ref-type="bibr" rid="r24"><italic>24</italic></xref>)</p>
<p>Veettil i suradnici (<xref ref-type="bibr" rid="r23"><italic>23</italic></xref>) nedavno su objavili rezultate presje&#x010D;nog istra&#x017E;ivanja provedenog na 569 zdravstvenih djelatnika zaposlenih u 14 zdravstvenih ustanova na razini PZZ-a u Dohi (Katar), u kojem su utvrdili da je prosje&#x010D;an broj to&#x010D;nih odgovora u trenutku istra&#x017E;ivanja bio 76,2%. Pritom su zdravstveni djelatnici s vi&#x0161;e radnog iskustva te oni koji su poha&#x0111;ali tri do &#x0161;est te&#x010D;aja KPR-a u zadnje tri godine imali statisti&#x010D;ki zna&#x010D;ajno vi&#x0161;e to&#x010D;nih odgovora. Sli&#x010D;ne rezultate smo utvrdili i u na&#x0161;em istra&#x017E;ivanju.</p>
<p>Ali i suradnici (<xref ref-type="bibr" rid="r22"><italic>22</italic></xref>) su 2019. godine objavili rezultate istra&#x017E;ivanja provedenog na 4.028 zdravstvenih djelatnika u Dubaiju (Ujedinjeni Arapski Emirati) koji su u razdoblju od 2010. do 2016. godine poha&#x0111;ali te&#x010D;aj KPR-a. Uspore&#x0111;ivali su to&#x010D;ne odgovore prije i nakon provedenog te&#x010D;aja. Prosje&#x010D;an postotak to&#x010D;nih odgovora na predispitu bio je 85% (uz standardnu devijaciju &#x00B1; 12), a na zavr&#x0161;nom ispitu 87% (uz standardnu devijaciju &#x00B1; 9), &#x0161;to je izra&#x017E;eno kao statisti&#x010D;ki zna&#x010D;ajno pove&#x0107;anje postotka to&#x010D;nih odgovora nakon provedenog te&#x010D;aja. Ovdje je va&#x017E;no naglasiti da su svi sudionici u tom istra&#x017E;ivanju dobili pripremne materijale u pisanom obliku prije nego &#x0161;to su polagali predispit! I mi smo poput navedenih istra&#x017E;iva&#x010D;a utvrdili signifikantan porast to&#x010D;nih odgovora na kontrolna pitanja nakon te&#x010D;aja organiziranog 2019. godine (58% vs 83%). Ipak, razli&#x010D;ito od navedenog istra&#x017E;ivanja, na&#x0161;i sudionici su pripremne materijale dobili tek nakon predaje predispita &#x2013; iz tog razloga smatramo da je postotak to&#x010D;nih odgovora na predispitu na&#x0161;ih sudionika bio signifikantno manji, ali je porast broja to&#x010D;nih odgovora na zavr&#x0161;nom ispitu bio zna&#x010D;ajnije ve&#x0107;i u odnosu na sudionike navedenog istra&#x017E;ivanja.</p>
<p>Po pitanju odaziva zdravstvenih djelatnika na redovite te&#x010D;ajeve KPR-a, i mi smo poput Sanchez Garcie i suradnika (<xref ref-type="bibr" rid="r25"><italic>25</italic></xref>) utvrdili da je taj odaziv nezadovoljavaju&#x0107;i. U na&#x0161;em istra&#x017E;ivanju &#x010D;ak vi&#x0161;e od 50% zdravstvenih djelatnika te&#x010D;ajeve ne ponavlja redovno u preporu&#x010D;enim intervalima svake dvije godine (<xref ref-type="table" rid="t1">Table 1</xref>), a svega 25 &#x2013; 30% ih se odaziva te&#x010D;ajevima organiziranim u na&#x0161;oj ustanovi. Smatramo da su za usporedno velike razlike po pitanju neredovitosti poha&#x0111;anja te&#x010D;ajeva KPR-a sudionika 2019. i 2022. godine odgovorna dva &#x010D;imbenika: 1. ostali domovi zdravlja koji su se 2020. godine pripojili Domu zdravlja Osijek u Dom zdravlja Osje&#x010D;ko-baranjske &#x017E;upanije svojim djelatnicima nisu redovno organizirali te&#x010D;ajeve KPR-a; 2. pandemija COVID-19 bila je odgovorna za nemogu&#x0107;nost provo&#x0111;enja adekvatnih te&#x010D;ajeva u periodu od 2020. do 2022. godine.</p>
<p>U na&#x0161;em istra&#x017E;ivanju se pokazalo da je po pitanju poznavanja rada AED-a i sigurnosti u postupak defibrilacije svega 50 &#x2013; 60% sudionika imalo pozitivan stav prije te&#x010D;ajeva, &#x0161;to je manje nego u istra&#x017E;ivanju koje su proveli Veettil i suradnici (<xref ref-type="bibr" rid="r23"><italic>23</italic></xref>), a u kojem je taj udio iznosio 81%. Smatramo da je razlog tomu neredovitost poha&#x0111;anja te&#x010D;ajeva KPR-a na&#x0161;ih sudionika. Ipak, nakon provedenog te&#x010D;aja 2019. godine utvrdili smo da se sigurnost u postupak defibrilacije pove&#x0107;ala na 95%.</p>
<p>S obzirom na to da se razdoblje izme&#x0111;u na&#x0161;a dva te&#x010D;aja poklopilo s razdobljem pandemije COVID-19 koja je prouzro&#x010D;ila nove strahove zdravstvenih djelatnika po pitanju transmisije bolesti tijekom postup- ka KPR-a (<xref ref-type="bibr" rid="r26"><italic>26</italic></xref>, <xref ref-type="bibr" rid="r27"><italic>27</italic></xref>) te na svjetskoj razini negativno utje-</p>
<p>cala na pre&#x017E;ivljenje OHCA i QoL unesre&#x0107;enih (<xref ref-type="bibr" rid="r28"><italic>28</italic></xref>&#x2013;<xref ref-type="bibr" rid="r30"><italic>30</italic></xref>), u anketi prije te&#x010D;aja 2022. godine dodali smo tri pita- nja koja su se odnosila na COVID-19. Prema odgovorima na njih dobili smo podatak da je deset (7%) sudionika bilo manje sigurno u postupak pravovremenog zapo&#x010D;injanja KPR-a &#x010D;ime smo, poput ostalih istra&#x017E;iva&#x010D;a, utvrdili da je i u na&#x0161;ih zdravstvenih djelatnika COVID-19 prouzro&#x010D;io odre&#x0111;eni strah i nesigurnost.</p>
<p>Na kraju &#x017E;elimo naglasiti da je na&#x0161;e istra&#x017E;ivanje imalo neke nedostatke. Prvo, radi se o retrospektivnoj studiji. Drugo, svega 19% sudionika te&#x010D;aja organiziranog 2022. godine bili su ponavlja&#x010D;i i samo na tom uzorku mogli smo adekvatno usporediti fluktuaciju to&#x010D;nih odgovora kroz razdoblje izme&#x0111;u dva te&#x010D;aja. Stoga &#x0107;e u budu&#x0107;nosti biti potrebno provesti prospektivno istra&#x017E;ivanje na ve&#x0107;em postotku ponavlja&#x010D;a.</p>
<p>Bez obzira na navedene nedostatke, smatramo da su rezultati na&#x0161;eg istra&#x017E;ivanja relevantni po pitanju situacije odaziva i teorijskog poznavanja postupka KPR-a kod zdravstvenih djelatnika na razini PZZ-a te da mogu poslu&#x017E;iti za daljnja istra&#x017E;ivanja i unaprje&#x0111;enje edukacije. S obzirom na dobivene rezultate, smatramo da bi te&#x010D;aj KPR-a trebali poha&#x0111;ati svi zdravstveni djelatnici u intervalima barem svake dvije godine. Pritom zdravstvene ustanove mogu pridonijeti pove&#x0107;anju broja djelatnika koji &#x0107;e te&#x010D;ajeve redovito poha&#x0111;ati na takav na&#x010D;in da ih periodi&#x010D;no same organiziraju i kontroliraju odaziv.</p>
</sec>
<sec sec-type="other5">
<title>Zaklju&#x010D;ci</title>
<p>Poznavanje pravilnog postupka KPR-a u zdravstvenih djelatnika s vremenom slabi, a redovitom reedukacijom smanjuje se fluktuacija znanja. Nakon godina iskustva u organizaciji te&#x010D;ajeva KPR-a utvrdili smo da se te&#x010D;ajevima odaziva svega 25 &#x2013; 30% od ukupnog broja zdravstvenih djelatnika na&#x0161;e ustanove, a manji postotak ih redovito ponavlja. Stoga smatramo da bi u Republici Hrvatskoj trebala postojati obveza redovitog poha&#x0111;anja te&#x010D;ajeva KPR-a za sve zdravstvene djelatnike, jer bi se samo na taj na&#x010D;in mogla odr&#x017E;avati zadovoljavaju&#x0107;a razina samopouzdanja, znanja i kvalitete postupka, &#x010D;ime bi se u kona&#x010D;nici doprinijelo redukciji smrtnih ishoda uslijed OHCA. Pritom bi zdravstvene ustanove trebale periodi&#x010D;no organizirati te&#x010D;ajeve svojim djelatnicima.</p>
