<title>SA&#x017D;ETAK</title>
<p>Kriptorhizam (nespu&#x0161;ten testis), izostanak jednog ili obaju testisa u normalnom skrotalnom polo&#x017E;aju, mo&#x017E;e biti kongenitalan ili ste&#x010D;en. Kongenitalni, primjetljiv ve&#x0107; pri ro&#x0111;enju, nastao je zastojem (intraabdominalno, ingvinalno ili visoko skrotalno) testisa na njegovom normalnom putu spu&#x0161;tanja. Kongenitalni kriptorhizam jedna je od naj&#x010D;e&#x0161;&#x0107;ih kongenitalnih anomalija (4% zdrave terminske novoro&#x0111;en&#x010D;adi i oko 45% nedono&#x0161;&#x010D;adi). Kriptorhi&#x010D;ni testisi, ako dugo ostanu u abnormalnom polo&#x017E;aju, prolaze kroz odre&#x0111;ene histolo&#x0161;ke promjene, a koje dovode do smanjene plodnosti i pove&#x0107;anog rizika za razvoj malignih tumora testisa. Stoga je preporuka kirur&#x0161;ko lije&#x010D;enje zavr&#x0161;iti do navr&#x0161;enih 12, najkasnije 18 mjeseci &#x017E;ivota. Cilj ovog rada bio je utvrditi pratimo li mi te smjernice. Prou&#x010D;avali smo razdoblje od 2011. do 2020. godine. U istra&#x017E;ivanje je uklju&#x010D;eno 598 djece, a koja su u vrijeme operacije bila u dobi od 0 do 18 godina &#x017E;ivota. Jednostrano nespu&#x0161;teni testis imalo je 413 (69,1%) djece, od &#x010D;ega 245 desno, a 168 lijevo. Bilateralni kriptorhizam je imalo 158 (26,4%) djece koja su operirana u istom aktu i jo&#x0161; 27 (4,5%) djece, kod kojih je prvo operirana jedna strana, a naknadno i druga. U prvih 16 mjeseci &#x017E;ivota operirano je 110 (18,4%) sve djece koja su analizirana u ovom radu. U prve tri godine &#x017E;ivota operirano je 279 (46,7%), a do polaska u &#x0161;kolu 431 (72%) djece. Prosje&#x010D;na dob djece pri obavljanju orhidopeksije bila je 57,8 mjeseci. Nema zna&#x010D;ajne razlike bilo da se radilo o jednostranom ili obostranom kriptorhizmu. Kroz &#x010D;itavo razdoblje pra&#x0107;enja (deset godina) nema razlike prosje&#x010D;ne starosti djece, niti se zamje&#x0107;uje tendencija pada. Valja zaklju&#x010D;iti kako se tek mali broj djece operira unutar prvih 18 mjeseci &#x017E;ivota, a zabrinjava izostanak tendencije smanjenja &#x017E;ivotne dobi u vrijeme zahvata u promatranom razdoblju.</p>

<title>SUMMARY</title>
<p>Cryptorchidism (undescended testicle), the absence of one or both testicles in the normal scrotal position, can be congenital or acquired. Congenital, noticeable already at birth, is caused by the stagnation (intra-abdominal, inguinal or high scrotal) of the testis on its normal path of descent. Congenital cryptorchidism is one of the most common congenital anomalies, occurring in 4% of healthy term newborns and about 45% of premature babies. Cryptorchid testicles, if they remain in an abnormal position for a long time, go through certain histological changes, which lead to reduced fertility and an increased risk for the development of malignant testicular tumors. Therefore, it is recommended to complete the surgical treatment by 12, at the latest 18 months of age. The aim of this paper was to determine whether we follow these guidelines. We studied the period from 2011 to 2020. A total of 598 children were included in this study, who were between 0 and 18 years old at the time of the operation. Out of the total number of children, 413 (69.1%) had unilateral undescended testicles, of which 245 were on the right and 168 on the left side. Bilateral cryptorchidism was present in 158 (26.4%) children who were operated on in the same act and another 27 (4.5%) children who were operated on one side first and then on the other., One hundred and ten(18.4%) children analyzed in this paper were operated in the first 16 months of life. In the first three years of life, 279 (46.7%) children were operated, and by the time they started school, 431 (72%) children. The average age of the children when orchidopexy was performed was 57.8 months. There is no significant difference considering whether it was unilateral or bilateral cryptorchidism. Throughout the entire monitoring period (ten years), there was no difference in the average age of the children, nor was there a tendency of decline. It can be concluded that only a small number of children are operated on within the first 18 months of life, and the absence of a tendency to decrease the age at the time of surgery in the observed period is worrying.</p>

<p>Kriptorhizam (nespu&#x0161;ten testis), izostanak jednog ili obaju testisa u normalnom skrotalnom polo&#x017E;aju, mo&#x017E;e biti kongenitalan ili ste&#x010D;en. Kongenitalni, primjetljiv ve&#x0107; pri ro&#x0111;enju, nastao je zastojem (intraabdominalno, ingvinalno ili visoko skrotalno) testisa na njegovom normalnom putu spu&#x0161;tanja. (<xref ref-type="bibr" rid="r1"><italic>1</italic></xref>) Kriptorhizam je &#x010D;esta kongenitalna anomalija. U 80 &#x2013; 85% slu&#x010D;ajeva je izolirana, znatno rje&#x0111;e je u sklopu nekog od gotovo 500 sindroma ili asocijacija, pri &#x010D;emu se mogu povezati s promjenama na 387 razli&#x010D;itih gena. (<xref ref-type="bibr" rid="r2"><italic>2</italic></xref>) Incidencija se razlikuje ovisno o promatranoj populaciji, a kod zdrave terminske novoro&#x0111;en&#x010D;adi iznosi 1 &#x2013; 4,6%. Testisi se mogu spontano spustiti tijekom prvih mjeseci &#x017E;ivota pa je incidencija u jednogodi&#x0161;nje djece manja i iznosi 1 &#x2013; 1,5%. U nedono&#x0161;&#x010D;adi je vi&#x0161;estruko &#x010D;e&#x0161;&#x0107;a i iznosi do 45% u trenutku ro&#x0111;enja, odnosno do 7,3% u dobi od jedne godine. (<xref ref-type="bibr" rid="r3"><italic>3</italic></xref>, <xref ref-type="bibr" rid="r4"><italic>4</italic></xref>) U razdoblju od 2016. do 2020. godine primarna zdravstvena za&#x0161;tita u Hrvatskoj je zabilje&#x017E;ila izme&#x0111;u 1.300 i 1.600 djece s kriptorhizmom. (<xref ref-type="bibr" rid="r5"><italic>5</italic></xref>&#x2013;<xref ref-type="bibr" rid="r9"><italic>9</italic></xref>) Ve&#x0107;ina djece ima jednostrani kriptorhizam (60 &#x2013; 70%), &#x010D;e&#x0161;&#x0107;e desno. U&#x010D;estalost polo&#x017E;aja nespu&#x0161;tenih testisa varira ovisno o istra&#x017E;ivanju, &#x0161;to je dijelom posljedica njihove razli&#x010D;ite klasifikacije. Naj&#x010D;e&#x0161;&#x0107;i polo&#x017E;aj je supraskrotalni, a intraabdominalni najrje&#x0111;i. (<xref ref-type="bibr" rid="r10"><italic>10</italic></xref>) U pro&#x0161;losti se smatralo kako su svi kriptorhizmi kongenitalni, ali danas postoje brojne studije koje opisuju ste&#x010D;eni kriptorhizam. Sekundarni kriptorhizam je ste&#x010D;eni kriptorhizam koji nastaje kada se testis podigne u supraskrotalni polo&#x017E;aj nakon hernioplastike ili kao posljedica orhidopeksije. Rekurentni kriptorhizam predstavlja testise koji su pri ro&#x0111;enju bili kriptorhi&#x010D;ni, tijekom vremena se spontano spustili, a zatim se opet podigli. Nestaju&#x0107;i testis (engl. <italic>vanishing testis</italic>) je testis koji je u po&#x010D;etku razvoja bio prisutan, ali je tijekom vremena izgubljen, naj&#x010D;e&#x0161;&#x0107;e kao posljedica nekog vaskularnog doga&#x0111;aja ili torzije. Ageneza testisa je stanje u kojem testis nije postojao ni u po&#x010D;etku razvoja. Retraktilni testisi su testisi koji se lako spontano povuku izvan skrotuma, ali se ru&#x010D;no mogu vratiti u skrotum, gdje ostaju barem privremeno. U ve&#x0107;ini je slu&#x010D;ajeva uzrok kriptorhizma nepoznat, ali danas postoje brojna istra&#x017E;ivanja koja kao uzrok navode djelovanje raznih genetskih i okoli&#x0161;nih &#x010D;imbenika. Najzna&#x010D;ajniji i nedvojbeno dokazan &#x010D;imbenik rizika je nedono&#x0161;enost. Tijekom vremena uo&#x010D;eno je i obiteljsko pojavljivanje kriptorhizma, sugeriraju&#x0107;i na vjerojatnost postojanja genetske podloge. Danas se istra&#x017E;uju brojni geni koji se smatraju etiolo&#x0161;kim &#x010D;imbenicima nesindromskog, izoliranog kriptorhizma. (<xref ref-type="bibr" rid="r10"><italic>10</italic></xref>) Okoli&#x0161;ni &#x010D;imbenici koji se naj&#x010D;e&#x0161;&#x0107;e povezuju s pojavom kriptorhizma jesu konzumacija cigareta i alkohola majke tijekom trudno&#x0107;e, maj&#x010D;in dijabetes, prekomjerna tjelesna masa i pretilost, konzumacija kofeina te izlo&#x017E;enost kemijskim tvarima koje djeluju kao endokrini disruptori. (<xref ref-type="bibr" rid="r11"><italic>11</italic></xref>) Posljedice kriptorhizma vidljive su histolo&#x0161;kim analizama kriptorhi&#x010D;nih testisa. Zamije&#x0107;ene su promjene ovisno o dobi u kojoj je u&#x010D;injena orhidopeksija. Tako su zna&#x010D;ajne razlike u histolo&#x0161;koj gra&#x0111;i testisa koji su spu&#x0161;teni u skrotum prije navr&#x0161;enih 12 mjeseci &#x017E;ivota u odnosu na one koji su operirani nakon te dobi. (<xref ref-type="bibr" rid="r12"><italic>12</italic></xref>) Najzna&#x010D;ajnije promjene uklju&#x010D;uju zaustavljen normalan razvoj stanica zametnog epitela i njihov smanjen broj u kriptorhi&#x010D;nom, ali u manjoj mjeri i u kontralateralnom testisu. Smanjen broj stanica povezan je, osim s odgo&#x0111;enom orhidopeksijom, i s nepalpabilnim testisima (intraabdominalna lokalizacija). Sli&#x010D;ne promjene zahva&#x0107;aju i Sertolijeve stanice &#x010D;iji je razvoj poreme&#x0107;en, a broj smanjen. Kriptorhi&#x010D;ni testisi imaju i smanjen promjer sjemenskih kanali&#x0107;a, izra&#x017E;eniju intersticijsku fibrozu te smanjen ukupni volumen. Sve ove promjene u korelaciji su s malignim aliteracijama tkiva testisa i smanjenjem plodnosti. (<xref ref-type="bibr" rid="r13"><italic>13</italic></xref>) Tako je rizik za razvoj malignog tumora testisa kod osoba koje su imale kriptorhizam u odnosu na zdravu populaciju tri puta ve&#x0107;i. Pored toga, zabilje&#x017E;ena je pove&#x0107;ana incidencija malignih tumora u kontralateralnim testisima. Va&#x017E;nu ulogu u razvoju ovih malignoma ima dob u kojoj se obavlja orhidopeksija. Istra&#x017E;ivanja na tu temu pokazala su do &#x0161;est puta ve&#x0107;i rizik kod dje&#x010D;aka operiranih nakon ulaska u pubertet u odnosu na one koji su operirani ranije. Tumori koji su najvi&#x0161;e povezani s kriptorhizmom jesu maligni tumori zametnog epitela testisa, &#x010D;e&#x0161;&#x0107;e seminomi. Utjecaj kriptorhizma na plodnost mu&#x0161;karaca ve&#x0107; je godinama tema brojnih istra&#x017E;ivanja. Ona su razli&#x010D;ito kategorizirala ispitanike prema dobi i zabilje&#x017E;ila vrlo &#x0161;irok raspon rezultata. Najbolje rezultate pokazale su orhidopeksije u&#x010D;injene prije navr&#x0161;ene druge godine &#x017E;ivota. Ako se usporedi op&#x0107;a populacija, gdje je stopa uspje&#x0161;nosti ostvarivanja biolo&#x0161;kog o&#x010D;instva 93%, kod mu&#x0161;karaca lije&#x010D;enih zbog unilateralnog kriptorhizma ona je ni&#x017E;a i iznosi oko 90%. No, kod mu&#x0161;karaca lije&#x010D;enih zbog bilateralnog kriptorhizma zna&#x010D;ajnije je ni&#x017E;a i iznosi oko 65%. (<xref ref-type="bibr" rid="r14"><italic>14</italic></xref>) &#x0160;to je dulja odgoda lije&#x010D;enja, ve&#x0107;i je i rizik za razvoj dugoro&#x010D;nih posljedica. Budu&#x0107;i da je spontano spu&#x0161;tanje testisa nakon navr&#x0161;enih &#x0161;est mjeseci &#x017E;ivota izuzetno rijetko, a uzev&#x0161;i u obzir podatke o histolo&#x0161;kim promjenama, lije&#x010D;enje kriptorhizma treba zapo&#x010D;eti u toj dobi. (<xref ref-type="bibr" rid="r15"><italic>15</italic></xref>) Bilo kakav oblik lije&#x010D;enja koji dovodi do normalnoga skrotalnog polo&#x017E;aja testisa treba zavr&#x0161;iti najkasnije do 18 mjeseci &#x017E;ivota. Lije&#x010D;enje kriptorhizma mo&#x017E;e biti konzervativno i kirur&#x0161;ko. Konzervativno lije&#x010D;enje kriptorhizma zasniva se na hormonalnoj ovisnosti prirodnog spu&#x0161;tanja testisa, a podrazumijeva primjenu hCG-a u obliku intramuskularnih injekcija ili GnRH-a u obliku nazalnog spreja. Uspje&#x0161;nost ovog na&#x010D;ina lije&#x010D;enja ovisi o prvotnom polo&#x017E;aju testisa &#x2013; &#x0161;to je on vi&#x0161;e polo&#x017E;en, uspje&#x0161;nost je slabija. Ona je, ukupno gledaju&#x0107;i, niska, do 20%, s rizikom za ponovno podizanje testisa u gotovo 20% slu&#x010D;ajeva. Studije koje su prou&#x010D;avale taj na&#x010D;in lije&#x010D;enja uglavnom su lo&#x0161;ije kvalitete, s heterogenim populacijama, razli&#x010D;itim polo&#x017E;ajima testisa, razli&#x010D;itim dozama i u&#x010D;estalostima primjene lijeka te oskudnim podatcima o dugoro&#x010D;nim rezultatima. Zbog svega toga, hormonsko lije&#x010D;enje nespu&#x0161;tenih testisa se ne preporu&#x010D;uje. Me&#x0111;utim, za dje&#x010D;ake s bilateralnim kriptorhizmom preporu&#x010D;uje se lije&#x010D;enje analozima GnRH-a s ciljem o&#x010D;uvanja njihovog reproduktivnog potencijala. (<xref ref-type="bibr" rid="r15"><italic>15</italic></xref>, <xref ref-type="bibr" rid="r16"><italic>16</italic></xref>) Kirur&#x0161;ko spu&#x0161;tanje testisa u normalni skrotalni polo&#x017E;aj naziva se orhidopeksija i danas je zlatni standard u lije&#x010D;enju kriptorhizma. (<xref ref-type="bibr" rid="r17"><italic>17</italic></xref>) Izvodi se u op&#x0107;oj anesteziji. U slu&#x010D;aju palpabilnog testisa, mo&#x017E;e se izvesti ingvinalnim ili skrotalnim pristupom. (<xref ref-type="bibr" rid="r18"><italic>18</italic></xref>) Naj&#x010D;e&#x0161;&#x0107;e se koristi ingvinalni pristup. Prvi korak u tom pristupu je rez u ingvinalnoj regiji, nakon &#x010D;ega se pristupa ingvinalnom kanalu. Skrotalni pristup se koristi kod nisko smje&#x0161;tenih palpabilnih kriptorhi&#x010D;nih testisa. Ako se radi o nepalpabilnim testisima, potreban je ponovni pregled u op&#x0107;oj anesteziji. Ako se tada testis mo&#x017E;e palpirati, radi se standardna ingvinalna orhidopeksija. Ako jo&#x0161; uvijek nije palpabilan, provodi se dijagnosti&#x010D;ka laparoskopija, s nekoliko mogu&#x0107;ih ishoda. U slu&#x010D;aju postavljanja dijagnoze nestaju&#x0107;eg (engl. <italic>vanishing</italic>) testisa, postupak je zavr&#x0161;en. U slu&#x010D;aju pronalaska intraabdominalno smje&#x0161;tenog testisa koji se nalazi previsoko za klasi&#x010D;nu ingvinalnu orhidopeksiju, provodi se Fowler-Stephensova orhidopeksija. Ona uklju&#x010D;uje ligaciju testikularnih krvnih &#x017E;ila kako bi se pobolj&#x0161;ala kolateralna cirkulacija. Nakon 6 &#x2013; 12 mjeseci one se presijeku i testis se fiksira u skrotumu. U slu&#x010D;aju pronalaska testisa u blizini unutarnjega ingvinalnog prstena, on se u skrotum mo&#x017E;e spustiti laparoskopski ili klasi&#x010D;nim ingvinalnim pristupom. Komplikacije orhidopeksije su rijetke i uklju&#x010D;uju uobi&#x010D;ajene postoperativne komplikacije &#x2013; infekcije rane, dehiscenciju rane, hematom. Specifi&#x010D;ne komplikacije su sekundarni kriptorhizam, ozljede krvnih &#x017E;ila i atrofija testisa. (<xref ref-type="bibr" rid="r19"><italic>19</italic></xref>) O problemu mu&#x0161;koga reproduktivnog zdravlja hrvatske djece iz aspekta dje&#x010D;jeg kirurga unatrag desetak godina pisali su &#x017D;upan&#x010D;i&#x0107;, Bahtijarevi&#x0107;, Todori&#x0107;. (<xref ref-type="bibr" rid="r20"><italic>20</italic></xref>&#x2013;<xref ref-type="bibr" rid="r22"><italic>22</italic></xref>) No, kod nas i dalje nema publiciranih radova koji obra&#x0111;uju i prate trend promjene dobi djece u kojoj je na&#x010D;injen operacijski zahvat nespu&#x0161;tenog testisa.</p>
<p>U ovom radu analizira se &#x017E;ivotna dob djece u vrijeme zahvata nespu&#x0161;tenog testisa kroz desetogodi&#x0161;nje razdoblje 2011. do 2020. godine.</p>
<sec sec-type="other1">
<title>Ispitanici i metode</title>
<p>Ukupno je 622 djece od po&#x010D;etka 2011. do kraja 2020. godine bilo primljeno u Zavod za dje&#x010D;ju kirurgiju, Klinike za kirurgiju Klini&#x010D;koga bolni&#x010D;kog centra Zagreb (u daljnjem tekstu Zavod) radi operativnog lije&#x010D;enja kriptorhizma. Svi podatci kori&#x0161;teni u ovom retrospektivnom istra&#x017E;ivanju prikupljeni su iz bolni&#x010D;koga informacijskog sustava KBC-a Zagreb. Prikupljeni i analizirani podatci uklju&#x010D;uju godinu u kojoj je pacijent operiran, dob pacijenta u vrijeme operacije izra&#x017E;enu u mjesecima te stranu na kojoj se nalazi nespu&#x0161;teni testis. Iz analize je isklju&#x010D;eno 24 djece (operacijski nalaz atrofije ili ageneze testisa, lije&#x010D;enje recidiva). Podatci su prikupljeni u prethodno sastavljene tablice, nakon &#x010D;ega su statisti&#x010D;ki analizirani pomo&#x0107;u programa <italic>Microsoft Office Excel</italic>. Dob pacijenata u vrijeme operacije analizirana je za svaku godinu posebno te za ukupno razdoblje od deset godina, i to posebno za jednostrano nespu&#x0161;tene testise, obostrano nespu&#x0161;tene testise, testise koji su bili nespu&#x0161;teni na obje strane, ali su operirani odvojeno, te ukupno za sve nespu&#x0161;tene testise.</p>
</sec>
<sec sec-type="other2">
<title>Rezultati</title>
<p>Prikupljeni su podatci 598 djece i adolescenata, &#x017E;ivotne dobi od 0 do 18 godina. Najmla&#x0111;e dijete operirano je u drugom danu &#x017E;ivota radi dijafragmalne hernije, tijekom &#x010D;ega je izveden i prvi akt orhidopeksije (lijevi testis, koji se nalazio u razini bubrega, fiksirao se za peritoneum u razini unutarnjega ingvinalnog prstena). Najstariji adolescent operiran je s punih 18 godina &#x017E;ivota, a radi retraktilnog testisa.</p>
<p>Jednostrano nespu&#x0161;teni testis imalo je 413 (69,1%) dje&#x010D;aka, od &#x010D;ega 245 desno, a 168 lijevo. Bilateralni kriptorhizam je imalo 158 (26,4%) dje&#x010D;aka koji su operirani u istom aktu i jo&#x0161; 27 (4,5%) njih kod kojih je prvo operirana jedna strana, a naknadno i druga. U prvih 12 mjeseci &#x017E;ivota operirano je 11 djece. U prvih 16 mjeseci &#x017E;ivota operirano je 110 djece, a jo&#x0161; njih 68 do navr&#x0161;ene dvije godine &#x017E;ivota. U tre&#x0107;oj godini &#x017E;ivota operirano je 101 dijete, a u tre&#x0107;oj, &#x010D;etvrtoj i petoj po njih 51. U godini polaska u &#x0161;kolu 52, u prvom razredu 31, u drugom razredu &#x0161;kole 28 dje&#x010D;aka. U kasnijoj &#x017E;ivotnoj dobi do punoljetnosti jo&#x0161; je operirano ukupno 55 djece. U prvih 16 mjeseci &#x017E;ivota operirano je 110 (18,4%) djece koja su analizirana u ovom radu. U prve tri godine &#x017E;ivota operirano je 279 (46,7%), a do polaska u &#x0161;kolu 431 (72%) djece.</p>
<p>Prosje&#x010D;na &#x017E;ivotna dob djece u vrijeme orhidopeksije iznosila je 58 mjeseci. U djece s jednostranim kriptorhizmom iznosila je prosje&#x010D;no 60 mjeseci, a u skupini s obostranim kriptorhizmom 54 mjeseca. Odnos dobi po godinama tijekom promatranog razdoblja prikazan je u <xref ref-type="table" rid="t1">Table 1</xref> za jednostrane, u <xref ref-type="table" rid="t2">Table 2</xref> za obostrane, a u <xref ref-type="table" rid="t3">Table 3</xref> za svu djecu.</p>

<p>Ako je orhidopeksija u&#x010D;injena na obje strane, ali ne u isto vrijeme, za izra&#x010D;un prosje&#x010D;ne dobi tijekom cijelog razdoblja od deset godina kori&#x0161;tena je dob pri prvoj orhidopeksiji.</p>
</sec>
<sec sec-type="other3">
<title>Rasprava</title>
<p>U razdoblju od 2011. do 2021. godine nema zna&#x010D;ajnih oscilacija broja operativno lije&#x010D;ene djece radi kriptorhizma, a koja su operativno lije&#x010D;ena u Zavodu. Samo je 18,4% djece operirano do navr&#x0161;enih 18 mjeseci &#x017E;ivota, a u prve &#x010D;etiri godine 63,7%. Dakle, ni prastare postulate kako svu djecu radi kriptorhizma treba operirati u prve &#x010D;etiri godine &#x017E;ivota nismo jo&#x0161; postigli. Iz na&#x0161;ih se rezultata vidi kako je ve&#x0107;ina djece operirana do prvog razreda osnovne &#x0161;kole (72%) i mo&#x017E;e se povezati sa sistematskim pregledom pred upis u &#x0161;kolu. Prosje&#x010D;na &#x017E;ivotna dob u vrijeme orhidopeksije u na&#x0161;oj skupini istra&#x017E;ivane djece bila je 58 mjeseci (4 godine i 10 mjeseci). Prosje&#x010D;na &#x017E;ivotna dob u vrijeme izvo&#x0111;enja orhidopeksije nije zna&#x010D;ajno varirala od godine do godine u promatranom vremenskom razdoblju. Nema niti trenda smanjivanja, ali niti pove&#x0107;avanja. To je uo&#x010D;ljivo ako se promatraju unilateralni, odnosno bilateralni kriptorhizam svaki zasebno, ali i zajedno. Ovaj realitetni problem u nas nije izoliran slu&#x010D;aj. Velik broj istra&#x017E;ivanja razli&#x010D;itih dr&#x017E;ava diljem svijeta opisuju sli&#x010D;an problem. Ve&#x0107;inom se prosje&#x010D;na dob djeteta prilikom izvo&#x0111;enja orhidopeksije u svijetu kre&#x0107;e izme&#x0111;u 24 i 64 mjeseca, s malim postotkom djece operirane u preporu&#x010D;enom razdoblju. (<xref ref-type="bibr" rid="r23"><italic>23</italic></xref>&#x2013;<xref ref-type="bibr" rid="r25"><italic>25</italic></xref>) U nekim istra&#x017E;ivanjima iznose se podatci da je izme&#x0111;u 27% i 35,2% djece operirano do navr&#x0161;enih 18 mjeseci &#x017E;ivota (<xref ref-type="bibr" rid="r26"><italic>26</italic></xref>) odnosno izme&#x0111;u 31% i 43% do navr&#x0161;ene dvije godine &#x017E;ivota. (<xref ref-type="bibr" rid="r27"><italic>27</italic></xref>) Ba&#x0161;kovi&#x0107; i suradnici analizirali su dob djece operirane u Klinici za dje&#x010D;je bolesti Klai&#x0107;eva, Zagreb. Analizirali su 198 djece koja su operirana 2019. godine, te je prosje&#x010D;na dob te djece bila 64,1 mjeseci. Kada se analiziraju samo djeca s kongenitalnim kriptorhizmom, njih 16% je operirano u dobi do 18 mjeseci &#x017E;ivota. Autori u radu konstatiraju kako je to prekasno. (<xref ref-type="bibr" rid="r28"><italic>28</italic></xref>) U nekim dr&#x017E;avama prosje&#x010D;na dob u vrijeme izvo&#x0111;enja orhidopeksije ima tendenciju smanjivanja, ali ona jo&#x0161; uvijek nije zadovoljavaju&#x0107;a. Tako Zvizdi&#x0107; i suradnici analiziraju samo vrijeme zahvata u djece s kongenitalnim kriptorhizmom u dva razdoblja, 2008. &#x2013; 2010. i 2015. &#x2013; 2017., te nalaze zna&#x010D;ajno smanjenje dobi, no i dalje je 65% djece operirano u dobi do 18 mjeseci &#x017E;ivota. (<xref ref-type="bibr" rid="r23"><italic>23</italic></xref>) U svijetu su provedena i brojna istra&#x017E;ivanja koja su prou&#x010D;avala razloge kasnog obavljanja orhidopeksije, a njihovi rezultati pokazuju &#x0161;irok spektar razloga. Kao jedan od naj&#x010D;e&#x0161;&#x0107;ih razloga navodi se kasno upu&#x0107;ivanje djeteta s kriptorhizmom dje&#x010D;jem kirurgu od strane lije&#x010D;nika primarne zdravstvene za&#x0161;tite. (<xref ref-type="bibr" rid="r29"><italic>29</italic></xref>) Taj podatak, nadalje, otvara nova pitanja o razlozima kasnog upu&#x0107;ivanja. Istra&#x017E;ivanja pokazuju da se jednim dijelom iza toga krije manjkavo znanje lije&#x010D;nika primarne zdravstvene za&#x0161;tite &#x2013; ne gledaju ili ne razumiju smjernice, ne razumiju patofiziologiju kongenitalnog nasuprot ste&#x010D;enom kriptorhizmu, nisu svjesni ozbiljnosti stanja, misle da &#x0107;e se s vremenom spustiti, predugo ih prate ili ih ne prate kontinuirano. (<xref ref-type="bibr" rid="r30"><italic>30</italic></xref>) Istra&#x017E;ivanje provedeno u SAD-u pokazalo je da 53% lije&#x010D;nika primarne zdravstvene za&#x0161;tite, koji su u kontaktu s pedijatrijskim pacijentima, nije imalo nikakav ili je imalo minimalan kontakt s dje&#x010D;jom urologijom tijekom edukacije. Kada se radi o pacijentima s obostrano nespu&#x0161;tenim testisima, vi&#x0161;e od 6% ispitanih lije&#x010D;nika nije educiralo roditelje ili je reklo da ne postoji pove&#x0107;an rizik za razvoj malignog tumora testisa, a vi&#x0161;e od 35% njih je isto u&#x010D;inilo s rizikom za smanjenu plodnost. Kada se radi o pacijentima s jednostrano nespu&#x0161;tenim testisom, taj udio iznosi vi&#x0161;e od 5% kod rizika za razvoj malignog tumora testisa, odnosno vi&#x0161;e od 35% kod rizika za smanjenu plodnost. U istom istra&#x017E;ivanju, 20% ispitanika reklo je da s upu&#x0107;ivanjem specijalistu &#x010D;eka do ulaska u pubertet. (<xref ref-type="bibr" rid="r31"><italic>31</italic></xref>) Drugo istra&#x017E;ivanje koje je provedeno me&#x0111;u studentima posljednje godine i lije&#x010D;nicima primarne zdravstvene za&#x0161;tite koji su u kontaktu s pedijatrijskim pacijentima pokazalo je kako 83% studenata, 12% pedijatara i 28% lije&#x010D;nika obiteljske medicine nikada nije vidjelo dijete s kriptorhizmom. Nadalje, 34% ispitanika nije znalo nabrojiti sve posljedice kriptorhizma, 68% nije znalo kada dijete treba uputiti specijalistu, 66% nije znalo do koje dobi dijete treba biti operirano, a 80% ih nije znalo do kada se to dijete treba pratiti. Lo&#x0161;ije rezultate uglavnom su davali mla&#x0111;i ispitanici. (<xref ref-type="bibr" rid="r32"><italic>32</italic></xref>) Jo&#x0161; jedno istra&#x017E;ivanje provedeno je me&#x0111;u specijalizantima pedijatrije, obiteljske medicine i urologije, a pokazalo je da njih 62% nije imalo nikakvu ili je imalo minimalnu formalnu edukaciju o pregledu skrotuma, 50% ih je to u&#x010D;ilo samostalno, a 20% ih je kriptorhizam definiralo pogre&#x0161;no. (<xref ref-type="bibr" rid="r33"><italic>33</italic></xref>) Za Hrvatsku ne postoje slu&#x017E;beni podatci o ovom pitanju. Neke studije kao razlog navode i lo&#x0161;e organiziranu primarnu zdravstvenu za&#x0161;titu te duge liste &#x010D;ekanja za operacije. <p>Ograni&#x010D;enje na&#x0161;eg istra&#x017E;ivanja je izostanak analize uzroka kasnog obavljanja orhidopeksije, odnosno kasnog upu&#x0107;ivanja dje&#x010D;jem kirurgiju (ascendiraju&#x0107;i i mobilni testisi, zanemarivanje djeteta, promakla dijagnoza&#x2026;).</p>
</sec>
<sec sec-type="other4">
<title>Zaklju&#x010D;ak</title>
<p>Kriptorhizam u dje&#x010D;joj dobi jedan je od uzroka mu&#x0161;ke neplodnosti. Rana dijagnostika i rano lije&#x010D;enje zna&#x010D;ajne su mjere prevencije. Pitanje mu&#x0161;koga reproduktivnog zdravlja hrvatske djece iz aspekta dje&#x010D;jeg kirurga i nespu&#x0161;tenih testisa, &#x017E;ivotne dobi u vrijeme operacije, ne prati preporuke stru&#x010D;nih udruga. Ovo pilot-istra&#x017E;ivanje treba pro&#x0161;iriti na razinu znanstvenog projekta i obuhvatiti sve zdravstvene ustanove u RH u kojima se lije&#x010D;e djeca nespu&#x0161;tenih testisa.</p>
