REKONSTRUKCIJA NOSNOG VRŠKA KOMBINIRANOM TEHNIKOM HRSKAVIČNIH DISKOVA I »ONLAY« UMETKA

Rinoplastika se općenito smatra najzahtjevnijim zahvatom u estetskoj kirurgiji. Nepravilnosti nošnog vrška utječu na smanjenje funkcije nosa, ali i na estetski izgled bolesnika, jer cjelokupni izraz lica uvelike ovisi o izgledu nošnog vrška. Kod 19 bolesnika učinjena je rekonstrukcija nosnog vrška kombiniranom tehnikom ekstrakorporalne septoplastike, umetanjem hrskavičnih diskova i postavljanjem »onlay« umetaka na hrskavične diskove. Kod 15 bolesnika radilo se o primarnoj rinoplastici, dok je kod četiri bolesnika operacija učinjena u svrhu korekcije primarne rinoplastike. Operacijski postupak primijenjen je kod bolesnika s: buloznim nosnim vrškom, bolesnika s rascjepom medijalnih i hipertrofijom lateralnih alarnih krura uz izrazito debelu ili pretjerano tanku kožu nosa. Opisana tehnika rekonstrukcije nosnog vrška pokazala je vrlo dobre rezultate, uz prihvatljiv postotak poslijeoperativnih komplikacija. Smatramo da se radi o metodi izbora za rekonstrukciju izrazitih nepravilnosti nosnog vrška, kod kojih uobičajene tehnike rekonstrukcije ne pokazuju zadovoljavajuće rezultate.

Summary. Rhinoplasty is often thought of as the most challenging of all aesthetic procedures. Irregularities of nasal tip impli- cate decreasing of nasal function and are generally regarded as an unattractive facial feature. The combined operative techniques which required extracorporeal septoplasty, incorporation of cartilage disc graft and onlay graft were performed in nineteen patients. Among the nineteen patients, fifteen had procedure of primary rhinoplasty and in four of them secondary or revised rhinoplasty was made. Indications for this operative technique were in patients with boxy nasal tip, bifid nasal tip, lateral alar tethering with extremely fat or thin nose skin. We emphasize that this technique is very successful with an acceptable percentage of postoperative complications. The tecnique is method of choice for reconstruction of extreme nasal tip irregularities where the usual reconstruction technique does not give satisfactory results.
