Bilateral meandering pulmonary vein – Case report with literature review

Summary
Meandering pulmonary vein (MPV) is an extremely rare congenital venous anomaly, with approximately 20 cases reported in the literature so far. It refers to a tortuous, aberrant pulmonary vein that most frequently drains the entire right lung or its part and empties into the left atrium. No treatment is required as there is no communication between the pulmonary and systemic circulation in the form of a left-to-right shunt. The most common differential diagnoses include Scimitar syndrome, pulmonary arteriovenous malformation, pulmonary venous varix, or a nodular lesion. The purpose of our study is to describe a very rare pulmonary venous anomaly that does not require treatment and to make a distinction between similar and more common venous anomalies that cause a left-to-right shunt. We present the case of a young patient with bilateral MPV detected incidentally during preoperative evaluation.
